After an acute coronary syndrome, a patient with a normal cholesterol level is prescribed a high-intensity statin. Why?
Four hours after cardiac surgery, the mediastinal drain, which had been draining steadily, suddenly stops. BP falls to 84/60 mmHg, the heart rate rises and the CVP climbs. What should the nurse suspect?
A patient with an acute stroke has a glucose of 15 mmol/L (270 mg/dL). They do not have known diabetes. What is appropriate?
A patient with recurrent diverticulitis reports passing air and faecal matter in the urine. What is the most likely cause?
Which is the correct technique for using a metered-dose inhaler without a spacer?
During a red cell transfusion, when are serious acute reactions most likely, so the nurse stays with the patient?
A patient with chronic kidney disease stage 4 has a high phosphate, a low-normal calcium and a very high parathyroid hormone (PTH). What does this show?
Which patient with diabetes should the nurse see first?
Six hours after surgery, a patient has not passed urine, feels the urge to go and has a firm swelling above the pubic bone. What is the most likely problem?
A · Statins reduce further events whatever the starting cholesterol
After ACS, high-intensity statin therapy reduces recurrent MI, stroke and death regardless of baseline cholesterol, partly by stabilising plaques. It is part of standard secondary prevention with antiplatelets, beta-blockers, ACE inhibitors and lifestyle change.
C · Cardiac tamponade from a clotted drain
A sudden stop in drainage with falling blood pressure, tachycardia and rising venous pressure suggests blood is collecting around the heart because the drain is blocked. Tamponade after cardiac surgery is an emergency: the surgeon is called at once and the chest may need to be reopened.
D · Report it and monitor glucose, as it can worsen outcome
Hyperglycaemia is common after stroke and is linked to worse outcomes. It is reported, monitored and treated in line with local protocols, while avoiding hypoglycaemia. The patient may also be tested for undiagnosed diabetes.
B · A fistula between the colon and the bladder
Pneumaturia and faecaluria suggest a colovesical fistula, a recognised complication of diverticulitis (and Crohn's disease or cancer). Recurrent urinary infections are common, and surgery is usually needed.
D · Breathe out, then breathe in slowly and deeply while pressing the canister, then hold
The patient breathes out, seals the lips around the mouthpiece, starts breathing in slowly, presses the canister and continues a deep breath, then holds it for about 10 seconds. Spacers make coordination easier.
B · In the first 15 minutes
Acute hemolytic and anaphylactic reactions usually appear early, so the transfusion is started slowly and the nurse stays with the patient for about the first 15 minutes, checking vital signs before, during and after according to policy.
D · Secondary hyperparathyroidism from CKD
In CKD, phosphate retention and reduced activation of vitamin D lower calcium, which stimulates the parathyroid glands. The result is high PTH with high phosphate, causing bone disease. Phosphate binders, diet and active vitamin D are used. Parathyroid Hormone (PTH) Test · MedlinePlus (NIH) (2023)
HCO₃⁻ = 21.4 mmol/L
PaCO₂ = 50.4 mmHg
A · Glucose 48 mg/dL (2.7 mmol/L), sweating and confused
Severe hypoglycemia with confusion can quickly lead to seizures and loss of consciousness. The other patients have problems that matter but are not immediately dangerous.
B · Urinary retention
An urge to void with a palpable bladder points to retention, not reduced urine production. The nurse confirms with a bladder scan if available and follows the prescribed plan, such as catheterization.
