B · Face the patient and speak clearly at a normal pace
Facing the patient in good light, reducing background noise and speaking clearly, without shouting, helps understanding. Hearing aids should be in and working.
C · Cool the burn under cool running water for about 20 minutes
Cool running water for about 20 minutes reduces pain and depth of injury. Ice can damage tissue further, grease holds heat in, and blisters are left intact to protect the skin.
C · Explain the risks, agree on ways to reduce them, respect the decision and document
A competent patient can accept a risk. The nurse explains the danger, offers measures such as a low bed, non-slip footwear, a clear path and a call light, respects the choice and documents the discussion.
C · Keeping the drainage bag below the level of the bladder
Keeping the bag below the bladder prevents backflow, and the closed system is not broken. Catheters are removed as soon as they are no longer needed.
A · Place the child in a knee-to-chest position
In a hypercyanotic (“tet”) spell, the knee-chest position raises systemic vascular resistance and sends more blood to the lungs. Oxygen and prescribed medication follow.
D · Sit down before using it, as it can cause dizziness and low blood pressure
GTN causes vasodilation, which can lead to headache, flushing, dizziness and hypotension. Patients should sit, and seek emergency help if the pain does not settle after the advised doses.
B · They may have no fever and present with confusion, falls or reduced mobility
Older people may have a normal or low temperature and present with delirium, falls or general decline. Early warning scores and a high index of suspicion are important.
D · A rise in blood glucose
Glucocorticoids raise blood glucose by increasing glucose production and reducing insulin sensitivity. They also tend to lower potassium, retain sodium and raise the white cell count by releasing neutrophils into the blood.
C · Pernicious anemia, treated with B12 injections
Pernicious anemia is an autoimmune condition in which antibodies block intrinsic factor or destroy the cells that make it, so B12 is not absorbed. Treatment is lifelong B12, usually by injection (very high oral doses are an alternative).
D · Use the salbutamol first, wait a few minutes, then use the steroid
The bronchodilator opens the airways so that the inhaled steroid can reach further into the lungs. A short wait between the inhalers is usually advised.
B · Cholinergic crisis from too much anticholinesterase drug
Excess acetylcholine causes muscarinic signs (salivation, sweating, miosis, diarrhoea) and depolarising weakness. In myasthenic crisis, weakness occurs without these cholinergic signs; both can cause respiratory failure.
C · Report it so occupational health can arrange follow-up screening
Contact tracing includes staff exposed without respiratory protection. Occupational health assesses the risk, checks for symptoms and arranges testing (for example IGRA) at suitable intervals. Treatment is offered for latent infection if found. Exposed people do not usually need to stop work unless they become unwell.
A · Nausea, diarrhoea and a slow heart rate
Donepezil is a cholinesterase inhibitor. Raised acetylcholine can cause nausea, vomiting, diarrhoea, vivid dreams and bradycardia, which can lead to dizziness or fainting.
B · The eye surgeon must be told, as it can cause floppy iris during surgery
Tamsulosin blocks alpha-1A receptors in the iris dilator muscle as well as in the prostate, which can cause intraoperative floppy iris syndrome during cataract surgery. The effect can persist after the drug is stopped, so the surgeon must know in advance to plan the operation. Whether to pause it is a decision for the team.
C · Their uterus contracts and relaxes more irregularly
In multiparas, and after overdistension such as twins, the uterus relaxes and contracts intermittently, causing stronger afterpains. Breastfeeding increases them through oxytocin.
D · Airborne precautions, ideally in a negative-pressure room
Measles spreads through the air and can remain in a room for up to about 2 hours. Airborne precautions, a negative-pressure room where available and immune staff are required.
B · Liver damage develops over days, so urgent acetylcysteine may still be needed
Paracetamol overdose may cause few early symptoms, but the toxic metabolite damages the liver over the following days. A timed blood level guides treatment with acetylcysteine, which works best when started early. A mental health assessment is also needed.
C · Mood changes caused by the corticosteroid
High-dose corticosteroids can cause mood changes, including mania, depression and psychosis. The nurse reports the change so the prescriber can review the dose and treatment.
B · Non-suicidal self-injury still increases the long-term risk of suicide, so risk is assessed each time
Self-harm is a strong predictor of later suicide, even when the person does not intend to die. Each episode needs a compassionate assessment of risk, triggers and the function of the behaviour.
D = 200 mcg/min
R = 25 gtt/min
Infusion rates: a pump is set in mL/h = volume/time in hours. A gravity drip is counted in drops per minute: gtt/min = volume (mL) × drop factor (gtt/mL)/time (min). The drop factor is printed on the tubing package: macrodrip sets deliver 10, 15 or 20 gtt/mL (blood sets often 10 gtt/mL in the US, 15 in the UK) and microdrip sets 60 gtt/mL. Always use the drop factor the question or package gives.
DBP = 62 mmHg
