A · They can affect pulse oximetry and checking skin colour
Dark varnish or false nails can block the oximeter's light and hide nail-bed colour. Clear access to the nails helps monitor oxygenation and circulation.
D · The systolic may be read too low and the diastolic too high
Fast deflation means the gauge drops past the first and last sounds before they are heard. The usual advice is to deflate at about 2–3 mmHg per second.
C · Within about 60 minutes before the incision
For most drugs, prophylaxis is given within 60 minutes before the incision so tissue levels are high during surgery. Some drugs, such as vancomycin, start earlier because they are infused over longer.
D · Give back blows and chest thrusts in turn
For infants under one year, the rescuer alternates sets of back blows with chest thrusts, with the head lower than the chest. Abdominal thrusts are not used in infants, and blind finger sweeps can push the object further in.
D · “I will lean on the crutch pads under my arms to rest.”
Leaning on the axillary pads can compress the nerves in the armpit. Weight should be taken through the hands, and the home should be made safe from trip hazards.
D · Urine output at least 0.5 mL/kg/h and clearer thinking
Improving urine output, mental state and heart rate show better tissue perfusion. A rising heart rate and cool skin suggest ongoing shock, and new crackles may mean fluid overload.
A · Avoid blood pressure readings and blood samples on that arm
The fistula arm is protected from pressure, cannulas and blood tests to prevent damage and clotting. Nurses check for a palpable thrill and audible bruit, which show it is working.
D · Respect the refusal, check it is informed and document it, and inform the team
An adult with capacity has the right to refuse treatment, even life-saving treatment. The team should make sure the patient understands the consequences, discuss alternatives such as cell salvage and record the decision.
A · Metformin
Metformin is cleared by the kidneys, and it is contraindicated when the eGFR is below 30 mL/min/1.73 m² because of the risk of lactic acidosis. Insulin doses may also need adjustment, but insulin is not contraindicated.
A · Low urine sodium and concentrated urine with a high specific gravity
In prerenal AKI the tubules are intact, so they keep sodium and water, giving a low urine sodium and concentrated urine. In ATN the damaged tubules cannot do this, so urine sodium is high, the urine is dilute and muddy brown casts appear. Urine specific gravity test · MedlinePlus (NIH) (2025)
D · The INR rises, so closer INR monitoring is needed
Metronidazole inhibits the metabolism of warfarin (mainly via CYP2C9), so warfarin levels and the INR rise. The bleeding risk increases, and the INR is checked more often during and after the course.
A · Explain that phantom limb pain is real and arrange treatment
Phantom limb pain is common after amputation and arises from changes in nerves and the brain. It is real and treated with analgesia, including drugs for neuropathic pain, plus measures such as mirror therapy. It does not mean infection.
C · Treat the fever, pain and possible dehydration, then reassess
Sinus tachycardia is usually a physiological response to fever, pain, hypovolaemia, anxiety, hypoxia or sepsis. Treating the cause (antipyretics, analgesia, fluids, oxygen if needed) is appropriate, while watching for sepsis. Rate-slowing drugs can be harmful by masking compensation.
D · Insulin moves potassium into cells; glucose prevents a hypo
Insulin activates the sodium–potassium pump, moving potassium into cells and lowering the serum level within about 15–30 minutes. Glucose is given to prevent hypoglycaemia. The effect is temporary, so potassium must also be removed.
A · Weak breathing at night, so a review for ventilation support
Weak respiratory muscles cause nocturnal hypoventilation and a rise in CO2, leading to morning headaches, disturbed sleep and daytime drowsiness. Non-invasive ventilation can improve symptoms and survival, in line with the patient's wishes.
A · Turn the patient onto their side to protect the airway
A drowsy patient who is vomiting may aspirate blood. Positioning on the side and suctioning protect the airway while help is called; large-bore IV access, bloods and fluid resuscitation follow.
A · Advise limiting caffeine to about 200 mg a day, roughly two cups of coffee
Guidelines advise limiting caffeine to about 200 mg a day in pregnancy because higher intakes are linked to low birth weight and miscarriage. Energy drinks and some teas also contain caffeine.
D · Obesity with low physical activity
Obesity and inactivity are the main modifiable risk factors for type 2 diabetes in young people. Family history and ethnicity also raise risk but cannot be changed. Healthy eating and activity are the basis of prevention and treatment.
B · "In summer she can drink less so she doesn't need the toilet at school."
Dehydration promotes sickling, so children with sickle cell disease need plenty of fluids, especially in hot weather and during exercise. Seeking help for fever, avoiding cold and checking the spleen are correct.
A · About 6–24 hours after the last drink
Minor withdrawal symptoms begin within hours, usually 6–24 hours after the last drink. Seizures are most likely in the first 1–2 days, and delirium tremens usually appears later, around 2–3 days.
D · "Relapse is common in recovery. Let's look at what led to it and what you learned."
Relapse is common and does not mean failure. Treating it as a learning opportunity, reviewing triggers and coping strategies and returning to the plan supports the person's motivation.
D = 166.7 mcg/min
R = 15 gtt/min
Infusion rates: a pump is set in mL/h = volume/time in hours. A gravity drip is counted in drops per minute: gtt/min = volume (mL) × drop factor (gtt/mL)/time (min). The drop factor is printed on the tubing package: macrodrip sets deliver 10, 15 or 20 gtt/mL (blood sets often 10 gtt/mL in the US, 15 in the UK) and microdrip sets 60 gtt/mL. Always use the drop factor the question or package gives.

按体重给药时,先求 units/h = units/kg/h × 体重。
D = 10.3 mcg/kg/min
按体重给药时,mcg/min = mcg/kg/min × 体重。然后 mL/h = mcg/min × 60/浓度。
DBP = 53 mmHg
HCO₃⁻ = 27.8 mmol/L
