The charge nurse overhears four instructions from an RN to a nursing assistant. Which one needs to be corrected?
Why is keeping a patient warm (normothermia) during surgery part of infection prevention?
An IV bag from the pharmacy holds the correct drug, but the label shows another patient’s name. What should the nurse do?
A patient has taken hydrochlorothiazide for several years. Which laboratory change is a recognized effect of this drug?
The nurse teaches a patient with severe neutropenia after chemotherapy. Which statement shows a need for further teaching?
Why is it important to prevent hypothermia during and after surgery?
A nurse teaches a patient with hypertension about their medicines. Which statement shows a need for further teaching?
Why are IV fluids given cautiously to a patient with heart failure?
A patient has just started levetiracetam. Which side effect should the nurse teach them to report?
A nurse teaches foot care to a patient with diabetic neuropathy. Which statement shows a need for further teaching?
Why is the "whoosh" test no longer used to check nasogastric tube position?
In week 5 of radiotherapy, the skin in the treatment field is broken, moist and painful. What should the nurse do?
A · “Check his glucose and give the insulin according to the sliding scale.”
Giving insulin is medication administration and needs a licensed nurse. Measuring glucose, hygiene and supervised walking are suitable tasks with clear reporting instructions.
D · Low body temperature reduces blood flow and immune function in the wound
Hypothermia causes vasoconstriction and less oxygen in the wound, which weakens the immune defense. Keeping the patient warm is linked to fewer surgical site infections.
C · Not hang it, and get a correctly labeled bag from the pharmacy
A label with another patient’s name means the bag may be meant for someone else or may differ in other ways. The nurse does not use it, returns it and reports the issue so the error can be investigated.
C · A rise in serum uric acid
Thiazides reduce uric acid excretion and can trigger gout. They also tend to lower potassium, sodium and magnesium, raise calcium (by reducing its excretion) and raise blood glucose and lipids. Hydrochlorothiazide · StatPearls (NCBI Bookshelf) (2026)
A · "I'll check my temperature rectally to be accurate"
Rectal temperatures, suppositories and enemas are avoided in neutropenia, because injury to the rectal mucosa can let bacteria into the blood. Prompt reporting of fever, hand hygiene and avoiding sick contacts are correct.
C · It increases the risk of wound infection, bleeding and cardiac complications
Inadvertent perioperative hypothermia impairs clotting and immune function and increases cardiac events and wound infection. Patients are warmed actively and their temperature monitored.
B · "I'll stop my tablets once my readings are normal."
Hypertension usually needs long-term treatment; stopping because readings are normal often makes blood pressure rise again. Patients should discuss any changes with their prescriber.
D · Extra volume can quickly cause pulmonary oedema
In heart failure, the ventricle cannot cope with large increases in preload, so IV fluids can raise filling pressures and cause pulmonary oedema. When fluids are needed, smaller volumes are given with close reassessment of breathing, crackles, JVP, weight and fluid balance.
D · Changes in mood, such as irritability, low mood or anger
Levetiracetam is widely used and has few drug interactions, but it can cause mood changes, irritability, depression and, rarely, suicidal thoughts. Patients and families should report these. Gum overgrowth is linked to phenytoin.
C · "I will buy corn plasters from the pharmacy for my corn."
Medicated corn plasters contain acids that can burn the skin and cause ulcers in people with neuropathy or poor circulation. Corns and calluses should be treated by a podiatrist.
A · Air can be heard even when the tube is in the lung
Auscultation of injected air cannot distinguish the stomach from the lungs or oesophagus, and misplaced tubes have caused deaths. pH testing of aspirate or X-ray is used instead.
A · Report it, keep it clean and use a non-adherent dressing
Moist desquamation is a more severe radiation skin reaction. The radiotherapy team is informed; the area is kept clean and covered with non-adherent dressings, avoiding adhesive on the skin, and analgesia is given. Infection is watched for.
