A patient has a nasogastric tube on suction. Which task can the nurse delegate to a nursing assistant?
A child is admitted with a suspected Wilms tumor (nephroblastoma). Which instruction is most important for all staff?
A patient restrained because of violent behavior is now calm, oriented and following instructions. What should the nurse do?
A patient with chronic kidney disease has a potassium of 5.6 mEq/L. Which food should the nurse advise the patient to limit?
An RN cares for a patient on a 24-hour urine collection. Which task can be delegated to unlicensed assistive personnel (UAP), such as a nursing or healthcare assistant?
Modern wound care generally aims to keep a healing wound bed:
A patient with hyperkalaemia is given IV insulin with glucose. What is the purpose, and what must be monitored?
Which side effect is particularly common with verapamil?
A patient with known left-sided lung cancer starts coughing up large amounts of fresh blood. While help is called, how should the patient be positioned?
A patient has erratic glucose readings. The nurse finds a soft, rubbery lump where they always inject. What is the most important teaching?
Why is it important to prevent perioperative hypothermia?
After signing the consent form, a patient tells the nurse, "I don't really understand what they are going to do." What should the nurse do?
B · Giving mouth care
Mouth care is routine and important because the patient cannot eat or drink. Checking position, irrigating the tube and assessing drainage need nursing skills and judgement.
D · Do not palpate the abdomen
Palpating a Wilms tumor can rupture the capsule and spread cancer cells. A sign is often put on the bed to warn staff not to palpate the abdomen.
B · Remove the restraints, because the reason for them has ended
Restraints are used only as long as needed to keep people safe. Once the patient no longer presents a danger, the restraints are removed, even if the order has not yet expired.
C · Bananas
Bananas, oranges, potatoes, tomatoes, dried fruit and beans are high in potassium. Apples, white rice and cabbage are lower-potassium choices that are commonly allowed on a potassium-restricted diet. Potassium in diet · MedlinePlus (NIH) (2026); Chronic kidney disease · MedlinePlus (NIH) (2025)
D · Saving all urine in the collection container as instructed
UAP can help with routine, standard tasks such as saving urine and keeping the container cool as instructed. Teaching, clinical decisions and interpreting results require nursing judgment and stay with the RN. 24-Hour Urine Collection and Analysis · StatPearls (NCBI Bookshelf) (2024)
D · Moist but not wet
A moist wound environment speeds epithelial cell migration and healing. Too much moisture causes maceration, so dressings are chosen to manage exudate.
B · To shift potassium into cells; blood glucose must be monitored for hypoglycaemia
Insulin drives potassium into cells, temporarily lowering serum levels. Glucose is given to prevent hypoglycaemia, but glucose monitoring is still needed for several hours.
A · Constipation
Verapamil is a rate-limiting calcium channel blocker that slows AV conduction. Constipation is its most common side effect. It can also cause bradycardia and should not be combined with beta-blockers without specialist care because of the risk of heart block.
A · Lying on the left side, with the bleeding lung down
In massive haemoptysis, the patient is positioned with the bleeding lung lowermost so blood does not flood the healthy lung. Oxygen, suction, IV access and urgent senior help follow. Treatment may include bronchial artery embolisation.
C · Use healthy sites and check glucose often, as less insulin may be needed
Lipohypertrophy causes poor and unpredictable absorption. When the patient switches to healthy tissue, more insulin is absorbed, so hypos can occur. Glucose is monitored closely, and doses may need to be reduced.
C · It increases bleeding, wound infection and heart complications
Even mild hypothermia impairs clotting, increases surgical site infection and cardiac events, and causes shivering and discomfort. Forced-air warming, warmed fluids and covering the patient help prevent it.
C · Inform the surgeon so that the procedure can be explained again
Valid consent requires understanding. The nurse advocates for the patient and asks the surgeon (who obtains consent) to explain again before surgery goes ahead. The conversation is documented.
