A nurse has a needlestick injury from a used needle. What should the nurse do first?
Which patient has the highest risk of a burn from a heating pad?
Over a few hours, the emergency department sees many previously healthy people with the same unusual severe symptoms. What should the nurse do?
A patient with recurrent gout starts allopurinol. Which laboratory result shows that the treatment is achieving its goal?
Which side effects are common after high doses of salbutamol (albuterol)?
Why should chronic hypernatraemia be corrected gradually?
A patient with heart failure starts dapagliflozin, an SGLT2 inhibitor. Which teaching is most relevant?
A patient who has had a total laryngectomy becomes breathless and needs oxygen. How should it be given?
Which finding most suggests fluid overload in a patient with chronic kidney disease?
A patient with myasthenia gravis is admitted with pneumonia. Which newly prescribed antibiotic should the nurse question?
A patient with a new colostomy asks what to eat at home. Which advice is most appropriate?
A few hours after an ERCP, a patient has severe epigastric pain radiating to the back, with vomiting. Which complication is most likely?
A · Wash the area with soap and water
The wound is washed with soap and water at once. The nurse then reports the injury immediately so the source patient can be tested and post-exposure treatment considered.
B · A patient with diabetic peripheral neuropathy
Reduced sensation, as in peripheral neuropathy, prevents a person from feeling heat damage. Patients with poor sensation, poor circulation or confusion need very close monitoring if heat is used at all.
B · Report it to infection prevention and public health authorities promptly
Clusters of similar unusual illness can signal an outbreak or bioterrorism. Early reporting allows public health to investigate, guide precautions and treatment, and protect the community.
C · A falling serum uric acid
Allopurinol reduces uric acid production by blocking xanthine oxidase. Over time, a lower serum uric acid reduces gout attacks and dissolves urate crystals. Kidney function is checked, and a rash must be reported because of rare severe skin reactions. Gout · StatPearls (NCBI Bookshelf) (2026)
Syndrome of inappropriate antidiuretic hormone secretion · MedlinePlus (NIH) (2025)
C · Tremor and a fast heart rate
Salbutamol is a beta2-agonist and can cause tremor, tachycardia and palpitations. High doses can also lower serum potassium.
D · Lowering the sodium too fast can cause cerebral oedema
In chronic hypernatraemia, brain cells adapt by holding extra solutes. If sodium falls too quickly, water moves into brain cells and causes cerebral oedema, so correction is controlled.
B · Keep the genital area clean, as fungal infections are more common
SGLT2 inhibitors increase glucose loss in the urine, raising the risk of genital thrush and urinary infections. They also cause mild diuresis, and in people with diabetes there is a rare risk of ketoacidosis, so they are paused during acute illness as advised. They improve outcomes in heart failure.
A · Via a mask or humidified device over the neck stoma
A total laryngectomy separates the trachea from the upper airway, so the patient breathes only through the neck stoma. Oxygen and any rescue breaths must be given via the stoma. These patients are "neck breathers" and often carry an alert card.
D · A weight gain of 2 kg (4.4 lb) in two days with ankle swelling
Rapid weight gain reflects fluid retention. Oedema, breathlessness, crackles in the lungs and a raised blood pressure are other signs. Daily weights are a simple and reliable way to track fluid balance.
D · Gentamicin
Aminoglycosides such as gentamicin can impair neuromuscular transmission and trigger a myasthenic crisis. Fluoroquinolones, magnesium and some other drugs carry similar warnings. The nurse checks with the prescriber before giving it.
B · Eat a normal, balanced diet and introduce foods gradually
People with a colostomy can usually eat a normal diet. Foods are reintroduced gradually to see how each one affects gas, odour and output, and regular meals and fluids help.
C · Post-ERCP pancreatitis
Pancreatitis is the most common serious complication of ERCP. Severe epigastric pain radiating to the back with raised amylase or lipase suggests it; management includes analgesia, IV fluids and close monitoring.
