A drowsy patient has an arterial blood gas with pH 7.30, PaCO₂ 58 mmHg (7.7 kPa) and HCO₃⁻ 25 mEq/L (mmol/L). Which problem is the priority?
After a head injury, a patient passes large amounts of very dilute urine, is thirsty and has a rising serum sodium. Which condition is most likely?
Where should the dispersive (grounding) pad for electrosurgery be placed?
During a disaster, the hospital needs to free beds quickly. Which patient is the best candidate for early discharge?
A woman with hyperemesis gravidarum has vomited for 3 days. Which urine finding would the nurse expect?
A patient has marked generalized edema. Results show heavy proteinuria, serum albumin 1.8 g/dL (18 g/L) and total cholesterol 340 mg/dL (8.8 mmol/L). Which condition fits best?
A patient with leukaemia has a very low platelet count. Which nursing precaution is appropriate?
A patient going home with a leg cast is taught about cast care. Which statement shows a need for further teaching?
A patient is on a short course of oral prednisolone for an asthma exacerbation. Which effect should the nurse monitor for?
A patient with severe aortic stenosis is prescribed glyceryl trinitrate (nitroglycerin) for chest pain. Why should the nurse use particular caution?
After transsphenoidal pituitary surgery, which advice should the patient follow?
A kidney transplant recipient has had vomiting and diarrhoea for two days. Why should the transplant team be contacted?
C · Poor ventilation causing carbon dioxide to build up
A low pH with a high PaCO₂ and normal bicarbonate is respiratory acidosis. The patient is not breathing out enough carbon dioxide, so the priority is to support ventilation and escalate.
A · Diabetes insipidus
Damage to the pituitary or hypothalamus can stop the release of antidiuretic hormone, causing diabetes insipidus. The patient loses water, so urine is dilute and sodium rises; SIADH causes the opposite.
A · On clean, dry skin over a well-perfused muscle, close to the surgical site
The pad returns the current safely. Good contact over a well-perfused muscle prevents burns, while bony areas, scars, hair and moisture reduce contact and can cause burns at the pad site.
D · A patient admitted overnight for observation who is now stable with normal tests
In a surge, stable patients who could be safely cared for at home are discharged first. Patients who are unstable or need close monitoring or titrated infusions must stay.
D · Ketones
Prolonged vomiting and poor intake lead to fat breakdown and ketonuria, along with dehydration and electrolyte loss such as hypokalemia. Urine ketones help assess severity and response to IV fluids and antiemetics. Ketones urine test · MedlinePlus (NIH) (2025)
D · Nephrotic syndrome
Nephrotic syndrome is heavy proteinuria with low albumin, edema and high lipids. Low albumin reduces oncotic pressure, causing edema, and the liver increases lipoprotein production. There is also a higher risk of clots and infection. Nephrotic syndrome · MedlinePlus (NIH) (2025)
D · Use a soft toothbrush and an electric razor
Thrombocytopenia increases bleeding risk. Gentle oral care, electric razors and avoiding IM injections and rectal procedures help prevent bleeding.
B · "I can push a knitting needle down the cast to scratch an itch."
Pushing objects under a cast can injure the skin and cause infection or pressure sores. Elevation, keeping the cast dry and reporting numbness, colour change or increasing pain are correct.
A · Raised blood glucose
Corticosteroids raise blood glucose, which matters especially in diabetes. Short courses can also cause mood changes, insomnia, indigestion and fluid retention. Long-term use adds osteoporosis, adrenal suppression and infection risk.
A · Vasodilators can cause a severe fall in BP with a fixed outflow obstruction
In severe aortic stenosis, cardiac output is fixed by the narrow valve. Vasodilators lower preload and afterload, and the heart cannot compensate, so the blood pressure can fall sharply and cause syncope. They are used with care and close monitoring.
C · Avoid blowing the nose, straining and bending forward
Raised pressure in the head can open the repair and cause a CSF leak. Patients avoid nose blowing, straining, coughing hard and bending forward, and sneeze with the mouth open. Straws may also be avoided.
D · The patient may not be absorbing their immunosuppressants
Vomiting and diarrhoea can reduce absorption of immunosuppressants, raising the risk of rejection, and dehydration can harm the graft. Tacrolimus levels can also rise with diarrhoea. The team may check levels and kidney function.
