A patient with schizophrenia says voices are telling him to hurt his roommate. What is the priority?
A patient is deteriorating quickly. The nurse has called the provider twice, but the provider has not come. What is the best action?
Which nursing care reduces the risk of ventilator-associated pneumonia?
A small-bore feeding tube has just been inserted blindly at the bedside. Which method is the most reliable way to confirm its position before the first feeding?
An older adult takes lisinopril and furosemide and recently started daily ibuprofen for knee pain. The creatinine has risen from 1.0 to 1.8 mg/dL (88 to 159 µmol/L). What is the most likely cause?
In suspected sepsis, what should ideally be collected before the first dose of antibiotics, as long as it does not delay treatment?
A patient's hypoglycaemia was caused by a sulfonylurea such as gliclazide or glipizide. Why might they need monitoring for longer than usual?
Why should a long-acting beta-2 agonist (LABA) never be used alone in asthma?
A patient receiving weeks of IV antibiotics through a PICC line in the left arm develops swelling and aching of that arm and neck veins that look fuller on the left. What should the nurse suspect?
Why is pethidine (meperidine) generally avoided for regular pain relief?
C · Keep everyone safe and tell the team, as the risk of harm is high
Command hallucinations to harm others are a serious safety risk. The nurse keeps the patient and others safe, for example by separating them, and informs the team at once.
B · Activate the rapid response team
Nurses can call the rapid response team directly when a patient is deteriorating. The nurse also informs the charge nurse and follows the chain of command about the provider’s lack of response.
B · Regular oral care to reduce bacteria in the mouth
Oral care, including tooth brushing, reduces the bacteria that can be aspirated around the endotracheal tube. It is part of ventilator care along with head elevation and sedation review.
A · By an x-ray reviewed by a competent clinician
An x-ray is the most reliable way to confirm the position of a newly placed blind-inserted feeding tube, and US guidance recommends it before first use. In the UK, testing a gastric aspirate (pH 5.5 or below) is the first-line check for nasogastric tubes, with x-ray used when no aspirate is obtained or the pH is too high. Both reject air auscultation and the bubble test, and a tube in the airway can still allow talking.
B · The NSAID reduces kidney blood flow on top of the other two drugs
NSAIDs reduce prostaglandin-driven blood flow to the kidney. Combined with an ACE inhibitor and a diuretic, this can cause acute kidney injury, especially in older adults. The nurse reports the result and the NSAID is usually stopped. Creatinine blood test · MedlinePlus (NIH) (2025)
Thiazide Diuretics · StatPearls (NCBI Bookshelf) (2025)
D · Blood cultures
Blood cultures taken before antibiotics are more likely to grow the organism, which allows targeted treatment later. Antibiotics should still be given promptly and must not be delayed.
B · Sulfonylureas act for a long time, so hypoglycaemia can recur
Sulfonylureas stimulate insulin release and have a long duration of action. Hypoglycaemia can be prolonged or recur, so glucose is monitored closely after treatment.
D · Without an inhaled steroid, it is linked with severe and fatal attacks
LABA monotherapy in asthma is linked with severe attacks and asthma deaths because it relieves symptoms while airway inflammation continues. In asthma, LABAs are always given with an inhaled corticosteroid, often in one combination inhaler.
C · A catheter-related deep vein thrombosis
Upper-limb DVT is a recognised complication of PICCs and other central lines. Arm swelling, pain and distended veins should be reported, and an ultrasound arranged. The line is not flushed forcefully, and treatment usually involves anticoagulation.
D · A metabolite can build up and cause seizures
Pethidine's metabolite norpethidine accumulates with repeated doses or kidney impairment and can cause agitation, tremor and seizures. Other opioids are preferred.
