Before a defibrillator shock is delivered, what must the person operating it do?
A hospital will introduce new infusion pumps and wants to find out how the process could fail before any harm occurs. Which method fits best?
The nurse teaches a patient with iron-deficiency anemia who is starting oral iron. Which statement shows a need for further teaching?
A spontaneously breathing patient is critically ill and hypoxic. Which oxygen delivery gives the highest concentration?
Why must blood samples for a cross-match be labelled at the bedside immediately after they are taken?
A patient who has had a total laryngectomy becomes breathless and needs oxygen. How should it be given?
When the nurse squeezes the trapezius muscle, the patient brings a hand up above the collarbone towards the stimulus. Which GCS motor score is this?
A patient has fever, confusion, odd behaviour and a new seizure. Herpes simplex encephalitis is suspected. Which drug is usually started straight away?
Which finding means a patient is usually ready to leave recovery for the ward?
A patient with suspected appendicitis says the pain suddenly eased, but an hour later has a temperature of 38.9 °C, a rigid abdomen and a pulse of 118/min. What does this suggest?
C · Call “all clear” and check that nobody is touching the patient or bed
Anyone touching the patient or bed could receive the shock. The operator announces “clear” and looks to confirm that nobody is in contact, and oxygen is moved away from the chest.
D · Failure mode and effects analysis (FMEA)
FMEA is a proactive method that looks at each step of a process, asks how it could fail and what the effect would be, and changes the process in advance. Root cause analysis is used after an event has happened.
Hematocrit Test · MedlinePlus (NIH) (2024); BUN (Blood Urea Nitrogen) · MedlinePlus (NIH) (2024)
D · "I'll take my iron tablet with a big glass of milk"
Calcium in milk, as well as antacids and tea, reduces iron absorption, so iron should not be taken with them. Vitamin C improves absorption, dark stools are expected, and iron overdose is a serious poisoning risk in young children. Iron deficiency anemia - Medical Encyclopedia · MedlinePlus (NIH) (2025); Iron in diet - Medical Encyclopedia · MedlinePlus (NIH) (2025)
D · Non-rebreathe mask with reservoir bag at 15 L/min
A non-rebreathe mask with a filled reservoir at 15 L/min delivers a high oxygen concentration and is used first in critical illness. The dose is then titrated down to the target saturation.
C · To prevent "wrong blood in tube" errors that can cause fatal reactions
A sample labelled with the wrong patient's details can lead to incompatible blood being issued. Labelling at the bedside after positive identification is a key transfusion safety step.
C · Via a mask or humidified device over the neck stoma
A total laryngectomy separates the trachea from the upper airway, so the patient breathes only through the neck stoma. Oxygen and any rescue breaths must be given via the stoma. These patients are "neck breathers" and often carry an alert card.
D · 5, localises to pain
Localising to pain (5) means a purposeful movement towards the source of pain. Withdrawal (4) is pulling away, abnormal flexion (3) is decorticate posturing, and obeying commands (6) needs no pain at all. A central stimulus, such as a trapezius squeeze, is used to test for localising.
A · IV aciclovir
Herpes simplex encephalitis is treatable but can cause severe brain damage. IV aciclovir is started as soon as it is suspected, without waiting for test results. Kidney function and hydration are monitored during treatment.
A · Awake, with a clear airway, stable observations and controlled pain
Discharge criteria include a protected airway, adequate breathing and oxygen saturation, stable vital signs, acceptable pain and nausea control, and no active bleeding, as set by local recovery criteria.
C · Perforation with peritonitis
Brief relief can occur when an inflamed appendix ruptures, followed by spreading peritonitis with fever, rigidity and tachycardia. This is a surgical emergency needing urgent review, IV fluids and antibiotics.
