Two weeks after birth, a mother says she hears voices telling her the baby is evil. What is the priority?
During a disaster, the hospital needs to free beds quickly. Which patient is the best candidate for early discharge?
A patient with a potassium of 2.8 mEq/L has potassium chloride prescribed IV. Which statement about giving it is correct?
A 5-year-old has new bruising and petechiae two weeks after a viral illness. Platelets are 12 × 10³/µL; hemoglobin, white cell count and clotting times are normal. Which condition is most likely?
Which statement about storing insulin is correct?
Why is hypokalaemia particularly dangerous in a patient taking digoxin?
A patient on spironolactone and an ACE inhibitor has a potassium of 6.8 mmol/L (6.8 mEq/L) with peaked T waves. What is usually given first to protect the heart?
Why is levodopa combined with carbidopa (or benserazide)?
A patient with a new arm fistula says their hand on that side is cold, pale and painful, especially during dialysis. What is the concern?
Which measure helps reduce the risk of delirium in an older surgical patient?
After forearm fracture surgery, a patient has severe pain not eased by opioids, made worse by passively moving the fingers. What should the nurse do?
C · Keep the mother and baby safe and arrange urgent psychiatric care
Hallucinations and delusions after birth suggest postpartum psychosis, a psychiatric emergency with risk to mother and baby. Safety and urgent specialist care are the priorities.
D · A patient admitted overnight for observation who is now stable with normal tests
In a surge, stable patients who could be safely cared for at home are discharged first. Patients who are unstable or need close monitoring or titrated infusions must stay.
B · It is always diluted and given by infusion pump, never as an IV push
Concentrated potassium chloride given as an IV push can cause fatal cardiac arrest, so it is never pushed. It is diluted, mixed well and given by infusion pump at a rate set by protocol, often with cardiac monitoring at higher rates.
B · Immune thrombocytopenia (ITP)
ITP is immune destruction of platelets, often after a viral infection in children. It causes isolated thrombocytopenia with normal other counts and clotting tests. Leukemia usually affects other cell lines, DIC prolongs clotting times and hemophilia prolongs the aPTT. Immune thrombocytopenic purpura (ITP) · MedlinePlus (NIH) (2026)
A · Unopened insulin is kept in the fridge and must not be frozen
Unopened insulin is refrigerated; in-use pens or vials are usually kept at room temperature for a limited time stated by the manufacturer. Frozen or overheated insulin must be discarded.
C · Low potassium increases the risk of digoxin toxicity
Digoxin binds to the sodium–potassium ATPase at the site potassium normally uses. When potassium is low, digoxin binds more, raising the risk of toxicity and arrhythmias.
需要尽快紧急再灌注,首选直接 PCI(如果不能及时进行 PCI 则溶栓);“时间就是心肌”。
B · Intravenous calcium gluconate or chloride
Severe hyperkalaemia with ECG changes is an emergency. IV calcium stabilises the myocardium within minutes. Insulin with glucose and nebulised salbutamol shift potassium into cells, and the cause (here spironolactone and the ACE inhibitor) is stopped. Cardiac monitoring is essential.
B · To stop levodopa being converted to dopamine outside the brain
Carbidopa blocks the enzyme that converts levodopa to dopamine in the periphery. More levodopa reaches the brain, so a lower dose works, and side effects such as nausea and low blood pressure are reduced.
D · Steal syndrome, which needs urgent review
In steal syndrome, the fistula diverts arterial blood from the hand, causing ischaemia: coldness, pallor, pain, numbness or weakness. It must be reported urgently, because severe cases can cause tissue loss.
A · Making sure they have their glasses and hearing aids
Delirium prevention includes using glasses and hearing aids, orientation, good sleep, hydration, nutrition, early mobilisation, pain control, avoiding unnecessary sedatives and involving family.
B · Loosen any tight dressing, keep the arm at heart level and escalate
These are early signs of compartment syndrome, where raised pressure in a muscle compartment reduces blood flow. Constrictive dressings are released, the limb is kept at heart level and urgent surgical review is needed, as fasciotomy may be required.
