1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: B · Two patients who both have C. difficile infection

Perché è questa la risposta

Cohorting patients with the same organism is acceptable when single rooms are not available. The other pairs would expose a vulnerable patient to an infection.

Risposta

B · Two patients who both have C. difficile infection

Risposta

D · Aspirare la tracheostomia
3.Controllo delle infezioni e sicurezza del paziente

Risposta: D · Mixing up pounds and kilograms can lead to large dosing errors

Perché è questa la risposta

Pediatric doses are often calculated per kilogram. A weight recorded in pounds but read as kilograms gives a dose more than twice as high, so weight is recorded and used only in kilograms.

Risposta

D · Mixing up pounds and kilograms can lead to large dosing errors
4.Controllo delle infezioni e sicurezza del paziente

Risposta: B · Report it at once, because fever with severe neutropenia is an emergency

Perché è questa la risposta

Febrile neutropenia can progress to sepsis quickly. The nurse reports it immediately so cultures can be taken and antibiotics started without delay.

Risposta

B · Report it at once, because fever with severe neutropenia is an emergency
5.Valori di laboratorio ed elettroliti

Risposta: D · Higher, about 2,5–3,5

Perché è questa la risposta

Mechanical heart valves, especially in the mitral position, have a high risk of thrombosis, so the INR target is usually higher, about 2,5–3,5. An INR of 5 or more carries a high bleeding risk.Verificato con: Label: WARFARIN SODIUM tablet · DailyMed (NLM/FDA) (2012)

Risposta

D · Higher, about 2,5–3,5
6.Valori di laboratorio ed elettroliti

Risposta: C · Saving all urine in the collection container as instructed

Perché è questa la risposta

UAP can help with routine, standard tasks such as saving urine and keeping the container cool as instructed. Teaching, clinical decisions and interpreting results require nursing judgment and stay with the RN.Verificato con: 24-Hour Urine Collection and Analysis · StatPearls (NCBI Bookshelf) (2024)

Risposta

C · Saving all urine in the collection container as instructed
7.Infermieristica clinica

Risposta: B · Check the patient for injuries before moving them

Perché è questa la risposta

After a fall, the patient is assessed first, including airway, breathing, circulation, head injury and possible fractures, before deciding how to move them safely. The fall is then reported and the care plan reviewed.

Risposta

B · Check the patient for injuries before moving them
8.Infermieristica clinica

Risposta: A · Loss of circulation to the hand as swelling builds under tight burned skin

Perché è questa la risposta

Circumferential full-thickness burns form a tight eschar that acts like a tourniquet as oedema develops. Neurovascular checks are needed, and escharotomy may be required.

Risposta

A · Loss of circulation to the hand as swelling builds under tight burned skin
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: A · Potassio e sodio bassi, acido urico e glucosio elevati

Risposta

A · Potassio e sodio bassi, acido urico e glucosio elevati
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · "I will measure it straight after my morning jog."

Perché è questa la risposta

Home readings should be taken after resting quietly for about 5 minutes, sitting with back supported, feet flat and the arm supported at heart level, with no recent exercise, caffeine or smoking. Two readings are often recorded. Measuring after exercise gives falsely high results.

Risposta

C · "I will measure it straight after my morning jog."
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: A · Worse on waking and on coughing, with vomiting

Perché è questa la risposta

Headaches from raised ICP are often worse on waking, when coughing, sneezing or bending, and may come with vomiting or visual changes. A new headache with these features needs prompt assessment.

Risposta

A · Worse on waking and on coughing, with vomiting
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: B · The other adrenal gland has been suppressed

Perché è questa la risposta

Excess cortisol from the adenoma suppressed ACTH, so the remaining adrenal gland is shrunken and underactive. Steroid replacement is needed until it recovers, which can take months. Stopping too early risks adrenal crisis.

Risposta

B · The other adrenal gland has been suppressed
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: B · To detect heparin-induced thrombocytopenia

Perché è questa la risposta

Heparin-induced thrombocytopenia is an immune reaction, usually 5–10 days after starting heparin, that lowers platelets and paradoxically causes thrombosis. Monitoring depends on risk and local policy.

Risposta

B · To detect heparin-induced thrombocytopenia
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: B · Do not use the tube until its position is confirmed by policy

Perché è questa la risposta

A change in external length means the tube may have moved. Pushing it back blindly is unsafe. It is not used until placement is confirmed by pH or X-ray, or it is replaced.

Risposta

B · Do not use the tube until its position is confirmed by policy
