1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: B · Glucose 48 mg/dL (2,7 mmol/L), sweating and confused

Perché è questa la risposta

Severe hypoglycemia with confusion can quickly lead to seizures and loss of consciousness. The other patients have problems that matter but are not immediately dangerous.

Risposta

B · Glucose 48 mg/dL (2,7 mmol/L), sweating and confused
2.Priorità e delega

Risposta: C · Respiratory muscle fatigue with possible impending respiratory failure

Perché è questa la risposta

Early in an asthma attack, fast breathing lowers the PaCO₂. A rising, “normal” value in a tiring patient means they can no longer move enough air, which is a warning sign of respiratory failure.

Risposta

C · Respiratory muscle fatigue with possible impending respiratory failure
3.Controllo delle infezioni e sicurezza del paziente

Risposta: A · Use a soft toothbrush and an electric razor

Perché è questa la risposta

With thrombocytopenia, minor trauma can cause serious bleeding. Bleeding precautions include soft toothbrushes, electric razors, avoiding rectal procedures and IM injections where possible, and preventing falls.

Risposta

A · Use a soft toothbrush and an electric razor
4.Controllo delle infezioni e sicurezza del paziente

Risposta: B · Use a professional medical interpreter

Perché è questa la risposta

Professional medical interpreters reduce errors and protect privacy. Children and other family members may translate wrongly, leave things out or be placed in a difficult position.

Risposta

B · Use a professional medical interpreter
5.Valori di laboratorio ed elettroliti

Risposta: C · Low potassium and low bicarbonate

Perché è questa la risposta

Lower gut fluid contains potassium and bicarbonate, so severe diarrhea causes hypokalemia and a normal anion gap (hyperchloremic) metabolic acidosis, plus dehydration. Vomiting, in contrast, causes metabolic alkalosis.Verificato con: Potassium Blood Test · MedlinePlus (NIH) (2024); Metabolic acidosis · MedlinePlus (NIH) (2025)

Risposta

C · Low potassium and low bicarbonate
6.Valori di laboratorio ed elettroliti

Risposta: A · Hemoglobin 6,4 g/dL (64 g/L) with new chest pain and a pulse of 124/min

Perché è questa la risposta

Severe anemia with chest pain and tachycardia suggests the heart is not receiving enough oxygen, which needs urgent action, often including transfusion. The other results are mildly abnormal but expected or stable.Verificato con: Anemia · StatPearls (NCBI Bookshelf) (2026)

Risposta

A · Hemoglobin 6,4 g/dL (64 g/L) with new chest pain and a pulse of 124/min
7.Infermieristica clinica

Risposta: B · In standard numbered positions across the front and left of the chest

Perché è questa la risposta

The six chest leads (V1–V6) have standard anatomical positions, from the 4th intercostal space beside the sternum to the mid-axillary line. Wrong placement can change the ECG and lead to misinterpretation.

Risposta

B · In standard numbered positions across the front and left of the chest
8.Infermieristica clinica

9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · Lying prone for periods and avoiding pillows under the stump

Perché è questa la risposta

Prolonged hip flexion, such as long periods sitting or a pillow under the stump, can lead to a fixed contracture that makes fitting a prosthesis difficult. Lying prone for periods, keeping the hip extended and early physiotherapy help prevent it.

Risposta

C · Lying prone for periods and avoiding pillows under the stump
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: A · Withhold the next doses and report urgently for liver tests

Perché è questa la risposta

Isoniazid, rifampicin and pyrazinamide can all cause hepatotoxicity. Jaundice, dark urine and nausea suggest drug-induced hepatitis, so the drugs are stopped and liver function is checked urgently. Orange urine from rifampicin does not cause yellow eyes. Alcohol increases the risk.

Risposta

A · Withhold the next doses and report urgently for liver tests
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: B · Ask questions that can be answered with yes, no or a gesture

Perché è questa la risposta

In expressive aphasia, comprehension is largely preserved. Short, closed questions, extra time, gestures and picture or word boards help. Shouting does not help, and constantly finishing sentences can frustrate the patient.

Risposta

B · Ask questions that can be answered with yes, no or a gesture
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: D · "I will put something between his teeth so he does not bite his tongue."

Perché è questa la risposta

Nothing should ever be put between the teeth during a seizure. Buccal midazolam is given between the gum and cheek, so the mouth does not need to be forced open. Timing the seizure and following the emergency care plan are correct.

Risposta

D · "I will put something between his teeth so he does not bite his tongue."
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · Less pain and a shorter hospital stay

Perché è questa la risposta

Smaller incisions usually mean less pain, faster mobilisation, fewer wound complications and shorter stays. There are still risks, including injury to organs and shoulder-tip pain from the gas.

Risposta

D · Less pain and a shorter hospital stay
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: A · Discuss reducing the fluid rate with the team

Perché è questa la risposta

An output of about 1,7 mL/kg/h suggests over-resuscitation. Excess fluid ("fluid creep") worsens oedema and can cause pulmonary oedema and compartment syndromes, so the rate is reviewed and reduced under the burn protocol.

Risposta

A · Discuss reducing the fluid rate with the team
