1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: C · Place the patient on a cardiac monitor

Perché è questa la risposta

Severe hyperkalemia can cause dangerous arrhythmias, so continuous cardiac monitoring comes first while the provider is informed and treatment is prepared.

Risposta

C · Place the patient on a cardiac monitor

Risposta: D · Wash hands often and do not share towels, pillows or eye drops

Perché è questa la risposta

Viral conjunctivitis spreads easily by contact with eye secretions. Hand hygiene, not touching the eyes and not sharing towels, pillows or eye products reduce spread.

Risposta

D · Wash hands often and do not share towels, pillows or eye drops

Risposta: B · Anti-D immunoglobulin (Rho(D) immune globulin)

Perché è questa la risposta

An Rh-negative mother can form anti-D antibodies if Rh-positive fetal blood enters her circulation, which can cause hemolytic disease in a later pregnancy. Anti-D immunoglobulin at about 28 weeks and after delivery of an Rh-positive baby prevents this sensitization.Verificato con: Red Blood Cell Antibody Screen · MedlinePlus (NIH) (2025); Hemolytic disease of the newborn · MedlinePlus (NIH) (2025)

Risposta

B · Anti-D immunoglobulin (Rho(D) immune globulin)
6.Valori di laboratorio ed elettroliti

Risposta: A · Iron overload, treated with iron chelation therapy

Perché è questa la risposta

The body has no active way to excrete iron, so repeated transfusions cause iron overload, which can damage the heart, liver and endocrine glands. A very high ferritin reflects this, and iron chelators are used to remove the excess.Verificato con: Thalassemia · MedlinePlus (NIH) (2026); Ferritin Blood Test · MedlinePlus (NIH) (2025)

Risposta

A · Iron overload, treated with iron chelation therapy
7.Infermieristica clinica

Risposta: B · Check the patient for injuries before moving them

Perché è questa la risposta

After a fall, the patient is assessed first, including airway, breathing, circulation, head injury and possible fractures, before deciding how to move them safely. The fall is then reported and the care plan reviewed.

Risposta

B · Check the patient for injuries before moving them
8.Infermieristica clinica

Risposta: B · A haematoma compressing the airway

Perché è questa la risposta

A rapidly expanding neck haematoma after thyroid surgery is an airway emergency. Call for urgent help; local protocols often require equipment to release the wound clips or sutures to be kept at the bedside.

Risposta

B · A haematoma compressing the airway
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: B · A water-based lubricant

Perché è questa la risposta

Oil-based products such as petroleum jelly can ignite or burn intensely in high oxygen concentrations and have caused facial burns. Water-based products are recommended. Oxygen cylinders are kept upright and secured, away from heat and flames.

Risposta

B · A water-based lubricant
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: A · Apply firm direct pressure and call for urgent surgical help

Perché è questa la risposta

Haemorrhage from a stump needs immediate direct pressure, elevation if appropriate, and urgent surgical help. A tourniquet should be available as policy directs. Vital signs are monitored for shock.

Risposta

A · Apply firm direct pressure and call for urgent surgical help
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: D · Weakness on the same side and loss of pain on the opposite side

Perché è questa la risposta

Damage to one side of the spinal cord, often from a stab wound, causes motor loss and loss of proprioception on the same side, and loss of pain and temperature on the opposite side. The pain fibres cross soon after entering the cord.

Risposta

D · Weakness on the same side and loss of pain on the opposite side
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: D · Urgent referral for assessment for mechanical thrombectomy

Perché è questa la risposta

IV thrombolysis is usually given within 4,5 hours of onset. Mechanical thrombectomy for a large vessel occlusion can be done later, typically up to 6 hours and in selected patients up to 24 hours, based on imaging. Time still matters, so referral is urgent.

Risposta

D · Urgent referral for assessment for mechanical thrombectomy
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: C · With every meal and snack

Perché è questa la risposta

Pancreatic enzymes digest fat, protein and carbohydrate, so they are taken with meals and snacks. The dose is adjusted to reduce steatorrhoea and support weight gain.

Risposta

C · With every meal and snack
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: A · Inform the surgical and anaesthetic teams, who will check the INR

Perché è questa la risposta

Recent warfarin may leave the INR too high for safe surgery or a spinal anaesthetic. The team needs to know at once; they check the INR and decide whether to delay or reverse.

Risposta

A · Inform the surgical and anaesthetic teams, who will check the INR
