1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: B · Stop the infusion and remove the cannula

Perché è questa la risposta

A cool, pale, swollen site suggests infiltration. The nurse stops the infusion, removes the cannula, elevates the limb and restarts the IV at another site as needed.

Risposta

B · Stop the infusion and remove the cannula
2.Priorità e delega

Risposta: B · A confused older adult at high risk of falls

Perché è questa la risposta

A confused patient at high risk of falls benefits from being close to staff, so they can be observed and helped quickly. The other patients are stable and independent.

Risposta

B · A confused older adult at high risk of falls
3.Controllo delle infezioni e sicurezza del paziente

Risposta: C · Decline, and give only medications that the nurse prepared or checked personally

Perché è questa la risposta

Nurses give only medicines they prepared themselves or verified fully, because they are accountable for what they give. A drug prepared by someone else may be wrong in ways the nurse cannot see.

Risposta

C · Decline, and give only medications that the nurse prepared or checked personally
4.Controllo delle infezioni e sicurezza del paziente

Risposta: B · Helping the patient bathe while wearing a gown and gloves

Perché è questa la risposta

Assistive personnel can carry out routine care for a stable patient, using the correct PPE. Assessment, teaching and clinical judgement, such as deciding when to stop precautions, cannot be delegated to them.

Risposta

B · Helping the patient bathe while wearing a gown and gloves
5.Valori di laboratorio ed elettroliti

Risposta: C · Give several deep-cough samples, often first thing in the morning

Perché è questa la risposta

Usually a series of three sputum samples is collected, at least one in the early morning, when secretions have pooled overnight. The patient rinses with water (not mouthwash), takes deep breaths and coughs deeply to bring up sputum, not saliva. Airborne precautions are used.Verificato con: Sputum Culture · MedlinePlus (NIH) (2024); Acid-Fast Bacillus (AFB) Tests · MedlinePlus (NIH) (2025)

Risposta

C · Give several deep-cough samples, often first thing in the morning
6.Valori di laboratorio ed elettroliti

Risposta: B · Calcium 7,2 mg/dL (1,8 mmol/L) with tingling lips

Perché è questa la risposta

Hypocalcemia after thyroid surgery, from parathyroid injury, can progress to tetany and laryngospasm. Tingling lips are an early sign, so the nurse assesses the airway and Chvostek and Trousseau signs and reports at once. The other results are expected after surgery.Verificato con: Calcium Blood Test · MedlinePlus (NIH) (2024)

Risposta

B · Calcium 7,2 mg/dL (1,8 mmol/L) with tingling lips
7.Infermieristica clinica

Risposta: A · Protamina solfato

Risposta

A · Protamina solfato
8.Infermieristica clinica

Risposta: A · Bacteria can grow in the blood at room temperature

Perché è questa la risposta

Red cells are stored cold to limit bacterial growth. Once out of controlled storage, the transfusion must be completed within the time limit to reduce the risk of bacterial contamination and transfusion-transmitted infection.

Risposta

A · Bacteria can grow in the blood at room temperature
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · Extra volume can quickly cause pulmonary oedema

Perché è questa la risposta

In heart failure, the ventricle cannot cope with large increases in preload, so IV fluids can raise filling pressures and cause pulmonary oedema. When fluids are needed, smaller volumes are given with close reassessment of breathing, crackles, JVP, weight and fluid balance.

Risposta

C · Extra volume can quickly cause pulmonary oedema
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: B · Cor pulmonale

Perché è questa la risposta

Chronic hypoxia in COPD causes pulmonary hypertension, which strains the right ventricle and leads to right heart failure with oedema and raised JVP. Management includes treating hypoxia (long-term oxygen if eligible), diuretics for oedema and optimising COPD treatment.

Risposta

B · Cor pulmonale
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: A · They increase, because dialysis removes amino acids

Perché è questa la risposta

Before dialysis, a moderate protein intake may be advised. Dialysis removes amino acids and protein, so needs rise, and malnutrition is a common risk. A renal dietitian sets individual targets.

Risposta

A · They increase, because dialysis removes amino acids
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: D · Secondary hyperparathyroidism from CKD

Perché è questa la risposta

In CKD, phosphate retention and low active vitamin D lower calcium, so the parathyroid glands release more PTH. This secondary hyperparathyroidism weakens bone. Treatment includes phosphate control, vitamin D and sometimes calcimimetics.

Risposta

D · Secondary hyperparathyroidism from CKD
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: C · Soon after an acute soft tissue injury, to reduce swelling

Perché è questa la risposta

Cold reduces swelling, bleeding and nerve conduction in acute injuries. It is wrapped and applied for short periods and avoided on areas with poor circulation or sensation. Heat is better for muscle spasm and stiffness.

Risposta

C · Soon after an acute soft tissue injury, to reduce swelling
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: A · Calcium gluconate gel

Perché è questa la risposta

Hydrofluoric acid penetrates deeply and binds calcium and magnesium, causing severe pain and possible life-threatening hypocalcaemia. Calcium gluconate gel binds fluoride; serum calcium and heart rhythm are monitored in larger exposures.

Risposta

A · Calcium gluconate gel
