1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: D · Glucose 48 mg/dL (2,7 mmol/L), sweating and confused

Perché è questa la risposta

Severe hypoglycemia with confusion can quickly lead to seizures and loss of consciousness. The other patients have problems that matter but are not immediately dangerous.

Risposta

D · Glucose 48 mg/dL (2,7 mmol/L), sweating and confused
2.Priorità e delega

Risposta: D · Stop the oxytocin infusion

Perché è questa la risposta

Late decelerations point to reduced blood flow through the placenta. Stopping oxytocin reduces contractions; the nurse then turns the patient onto her side, gives IV fluids as prescribed and informs the provider.

Risposta

D · Stop the oxytocin infusion
3.Controllo delle infezioni e sicurezza del paziente

Risposta: A · Within about 60 minutes before the incision

Perché è questa la risposta

For most drugs, prophylaxis is given within 60 minutes before the incision so tissue levels are high during surgery. Some drugs, such as vancomycin, start earlier because they are infused over longer.

Risposta

A · Within about 60 minutes before the incision
4.Controllo delle infezioni e sicurezza del paziente

Risposta: D · There are too few neutrophils to produce the usual inflammatory signs

Perché è questa la risposta

Classic signs of infection come from the inflammatory response, which depends on white blood cells. With very few neutrophils, fever may be the only early sign, so it must be taken seriously.

Risposta

D · There are too few neutrophils to produce the usual inflammatory signs
5.Valori di laboratorio ed elettroliti

Risposta: B · Diabete insipido

Risposta

B · Diabete insipido
6.Valori di laboratorio ed elettroliti

Risposta: D · Have the patient wash and dry their hands, then repeat the test

Perché è questa la risposta

Sugar residue on the fingers can give a falsely high finger-stick result. The hands are washed with soap and water and dried, and the test repeated. If the value is still high, it is confirmed and reported. Treating a false result could cause hypoglycemia.

Risposta

D · Have the patient wash and dry their hands, then repeat the test
7.Infermieristica clinica

Risposta: C · The local anaesthetic can block sympathetic nerves and cause hypotension

Perché è questa la risposta

Epidural local anaesthetic can cause vasodilation and hypotension. Nurses also monitor sedation, respiratory rate, block height, motor function and the catheter site.

Risposta

C · The local anaesthetic can block sympathetic nerves and cause hypotension
8.Infermieristica clinica

Risposta: B · Medicines reconciliation against reliable sources, such as primary care records

Perché è questa la risposta

Medicines reconciliation compares several sources to produce an accurate medication list. It finds omissions, duplications and errors, and is especially important in older people with polypharmacy.

Risposta

B · Medicines reconciliation against reliable sources, such as primary care records
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: B · Quickly and deeply, after breathing out away from the device

Perché è questa la risposta

Dry powder inhalers have no propellant, so a fast, deep inhalation is needed to disperse the powder. The patient breathes out away from the device first, because moist breath can clump the powder. Very weak or breathless patients may not manage this.

Risposta

B · Quickly and deeply, after breathing out away from the device
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: D · The patient cannot breathe out and may suffocate

Perché è questa la risposta

A speaking valve opens on inspiration and closes on expiration, so exhaled air must pass around the tube and up through the larynx. If the cuff is inflated, air cannot escape, causing air trapping and respiratory arrest. The cuff must be fully deflated first.

Risposta

D · The patient cannot breathe out and may suffocate
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: A · Keep regular sleep and take every dose on time

Perché è questa la risposta

Missed doses and lack of sleep are among the most common seizure triggers. Excess alcohol and, in some people, flashing lights are others. Most people can exercise safely, with sensible precautions such as not swimming alone.

Risposta

A · Keep regular sleep and take every dose on time
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: B · Look for causes such as low oxygen, pain, retention or infection

Perché è questa la risposta

Acute confusion after surgery is often delirium. The nurse first looks for causes: hypoxia, pain, retention, constipation, infection, low glucose, dehydration and drugs. Sedation and restraints can make it worse and are a last resort.

Risposta

B · Look for causes such as low oxygen, pain, retention or infection
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: B · An underweight patient who has eaten very little for 10 days

Perché è questa la risposta

Risk factors include a low BMI, little or no intake for over 5 days, significant weight loss, alcohol misuse and low electrolytes before feeding. These patients start nutrition slowly with electrolyte monitoring.

Risposta

B · An underweight patient who has eaten very little for 10 days
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · Reduced intake, mottled skin and changes in breathing

Perché è questa la risposta

Signs include increasing drowsiness, reduced intake, mottled cool peripheries, changes in breathing such as pauses, and noisy secretions. Recognising them allows care to focus on comfort and family support.

Risposta

D · Reduced intake, mottled skin and changes in breathing
