1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: A · Persona giusta

Risposta

A · Persona giusta
2.Priorità e delega

Risposta: A · Respiratory muscle fatigue with possible impending respiratory failure

Perché è questa la risposta

Early in an asthma attack, fast breathing lowers the PaCO₂. A rising, “normal” value in a tiring patient means they can no longer move enough air, which is a warning sign of respiratory failure.

Risposta

A · Respiratory muscle fatigue with possible impending respiratory failure

Risposta

A · Il lembo più lontano dall'infermiere
4.Controllo delle infezioni e sicurezza del paziente

Risposta: D · No hepatitis B treatment is needed, but the injury is still reported and followed up

Perché è questa la risposta

A documented vaccine responder is protected against hepatitis B and needs no hepatitis B prophylaxis. The exposure is still reported, because the nurse and source patient need assessment for HIV and hepatitis C as well.

Risposta

D · No hepatitis B treatment is needed, but the injury is still reported and followed up
5.Valori di laboratorio ed elettroliti

Risposta: B · 0,9% sodium chloride

Perché è questa la risposta

Only 0,9% sodium chloride is used with red cells. Dextrose solutions can cause hemolysis, and lactated Ringer's contains calcium, which can cause clotting in the citrated blood. Drugs are never added to blood products.

Risposta

B · 0,9% sodium chloride
6.Valori di laboratorio ed elettroliti

Risposta: D · Increasingly severe pancreatitis

Perché è questa la risposta

Rising BUN (fluid loss and kidney injury), low calcium and hyperglycemia are markers of more severe pancreatitis in scoring systems. The nurse monitors fluid balance, vital signs and organ function closely and reports deterioration.Verificato con: Acute pancreatitis · MedlinePlus (NIH) (2025); BUN (Blood Urea Nitrogen) · MedlinePlus (NIH) (2024)

Risposta

D · Increasingly severe pancreatitis
7.Infermieristica clinica

Risposta: B · Occlusione intestinale

Perché è questa la risposta

L'occlusione intestinale causa distensione, dolore colico, vomito e chiusura dell'alvo a feci e gas.

Risposta

B · Occlusione intestinale
8.Infermieristica clinica

Risposta: C · To prevent "wrong blood in tube" errors that can cause fatal reactions

Perché è questa la risposta

A sample labelled with the wrong patient's details can lead to incompatible blood being issued. Labelling at the bedside after positive identification is a key transfusion safety step.

Risposta

C · To prevent "wrong blood in tube" errors that can cause fatal reactions
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · Lying prone for periods and avoiding pillows under the stump

Perché è questa la risposta

Prolonged hip flexion, such as long periods sitting or a pillow under the stump, can lead to a fixed contracture that makes fitting a prosthesis difficult. Lying prone for periods, keeping the hip extended and early physiotherapy help prevent it.

Risposta

C · Lying prone for periods and avoiding pillows under the stump
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: D · Check mask fit and protect the bridge of the nose

Perché è questa la risposta

Pressure injury over the bridge of the nose is common with NIV masks. Correctly sized masks, avoiding overtightening (allowing a small gap under the straps), protective dressings and regular skin checks reduce it. Other problems include eye irritation, dryness and gastric distension.

Risposta

D · Check mask fit and protect the bridge of the nose
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: B · Venous thrombosis

Perché è questa la risposta

In nephrotic syndrome, anticoagulant proteins such as antithrombin are lost, and clotting factors made by the liver rise, so venous thrombosis is more likely. Infection and high cholesterol are other complications.

Risposta

B · Venous thrombosis
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: D · The eye surgeon must be told, as it can cause floppy iris during surgery

Perché è questa la risposta

Tamsulosin blocks alpha-1A receptors in the iris dilator muscle as well as in the prostate, which can cause intraoperative floppy iris syndrome during cataract surgery. The effect can persist after the drug is stopped, so the surgeon must know in advance to plan the operation. Whether to pause it is a decision for the team.

Risposta

D · The eye surgeon must be told, as it can cause floppy iris during surgery
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: B · Stopping smoking

Perché è questa la risposta

Smoking worsens Crohn's disease, increasing flares, complications and the need for surgery. Stopping smoking is one of the most effective changes a patient can make.

Risposta

B · Stopping smoking
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: C · An intramuscular injection

Perché è questa la risposta

IM injections can cause haematomas in severe thrombocytopenia. Rectal procedures, NSAIDs and aspirin are also avoided. The nurse checks with the prescriber and the oral or IV route is used where possible.

Risposta

C · An intramuscular injection
