1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: B · Indicazioni e comunicazione giuste

Risposta

B · Indicazioni e comunicazione giuste
2.Priorità e delega

Risposta: C · Standard precautions, because it does not usually spread from person to person

Perché è questa la risposta

Inhalational anthrax is not spread from person to person, so standard precautions are used. Contact precautions may be added for skin (cutaneous) anthrax with draining lesions.

Risposta

C · Standard precautions, because it does not usually spread from person to person
3.Controllo delle infezioni e sicurezza del paziente

Risposta: B · Use the oral syringe or dosing cup that comes with the medicine

Perché è questa la risposta

Household spoons vary widely in size and cause dosing errors. A marked oral syringe or dosing device in mL gives an accurate dose.

Risposta

B · Use the oral syringe or dosing cup that comes with the medicine
4.Controllo delle infezioni e sicurezza del paziente

Risposta: B · “If the wound gets red and drains pus, I will wait for my appointment next month.”

Perché è questa la risposta

Increasing redness, purulent drainage, fever and worsening pain are signs of surgical site infection and should be reported promptly. Waiting a month could allow the infection to spread.

Risposta

B · “If the wound gets red and drains pus, I will wait for my appointment next month.”
5.Valori di laboratorio ed elettroliti

Risposta: C · After about 6–8 weeks

Perché è questa la risposta

Levothyroxine has a half-life of about a week, so a steady state takes about 6 weeks. The TSH is therefore rechecked about 6–8 weeks after a dose change; checking earlier gives a misleading result.Verificato con: Hypothyroidism · MedlinePlus (NIH) (2024)

Risposta

C · After about 6–8 weeks
6.Valori di laboratorio ed elettroliti

Risposta: D · IV hydration, a uric acid-lowering drug and frequent electrolytes

Perché è questa la risposta

Prevention includes aggressive IV hydration, drugs such as allopurinol or rasburicase to lower uric acid, and frequent monitoring of potassium, phosphate, calcium, uric acid and creatinine. Tumor lysis usually begins within 1–3 days of starting treatment.Verificato con: Uric Acid Test · MedlinePlus (NIH) (2026)

Risposta

D · IV hydration, a uric acid-lowering drug and frequent electrolytes
7.Infermieristica clinica

Risposta: C · Attacks of symptoms, such as visual loss or numbness, that improve between relapses

Perché è questa la risposta

MS causes demyelination in the brain and spinal cord. Many patients first have attacks such as optic neuritis or sensory symptoms, with partial or full recovery between relapses.

Risposta

C · Attacks of symptoms, such as visual loss or numbness, that improve between relapses
8.Infermieristica clinica

Risposta: C · Disseminated intravascular coagulation

Perché è questa la risposta

In DIC, widespread activation of clotting consumes platelets and clotting factors, causing both small clots and bleeding. Treating the underlying cause and replacing blood products as needed are key.

Risposta

C · Disseminated intravascular coagulation
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · Ambulatory or structured home blood pressure monitoring

Perché è questa la risposta

"White-coat" hypertension means raised readings only in clinical settings. Ambulatory monitoring over 24 hours or a structured series of home readings gives a truer picture and is widely recommended before diagnosing hypertension.

Risposta

C · Ambulatory or structured home blood pressure monitoring
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: D · Question the prescription, as adenosine can cause bronchospasm

Perché è questa la risposta

Adenosine can trigger severe bronchospasm and is generally avoided in asthma. Alternatives such as verapamil may be used, depending on the patient. The nurse raises the concern with the prescriber before giving it.

Risposta

D · Question the prescription, as adenosine can cause bronchospasm
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: B · Changes in mood, such as irritability, low mood or anger

Perché è questa la risposta

Levetiracetam is widely used and has few drug interactions, but it can cause mood changes, irritability, depression and, rarely, suicidal thoughts. Patients and families should report these. Gum overgrowth is linked to phenytoin.

Risposta

B · Changes in mood, such as irritability, low mood or anger
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: B · A patient on insulin for DKA whose potassium is now 2,9 mmol/L

Perché è questa la risposta

Insulin drives potassium into cells, and a potassium of 2,9 mmol/L during DKA treatment can cause life-threatening arrhythmias. The insulin rate and potassium replacement need urgent review under the protocol. The other patients are stable.

Risposta

B · A patient on insulin for DKA whose potassium is now 2,9 mmol/L
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · A narrowing such as an oesophageal tumour

Perché è questa la risposta

Progressive dysphagia, solids first then liquids, with weight loss, is typical of a mechanical obstruction such as oesophageal cancer or a stricture. It needs an urgent referral for endoscopy.

Risposta

D · A narrowing such as an oesophageal tumour
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: A · Treat it as a possible burn, report it and complete an incident form

Perché è questa la risposta

A blister under the return electrode suggests a diathermy burn. The nurse assesses and dresses it, informs the team, and reports it as an incident so that the cause can be investigated.

Risposta

A · Treat it as a possible burn, report it and complete an incident form
