1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: A · Increasing sleepiness that is hard to rouse

Perché è questa la risposta

Sedation usually increases before breathing slows. Regular sedation checks let the nurse hold the opioid and act before respiratory depression occurs.

Risposta

A · Increasing sleepiness that is hard to rouse
2.Priorità e delega

Risposta: A · An LPN/LVN is assigned to admit a new patient from the emergency department

Perché è questa la risposta

Admitting a new patient includes the initial assessment and plan of care, which belong to the RN. The other assignments match each staff member’s role.

Risposta

A · An LPN/LVN is assigned to admit a new patient from the emergency department
3.Controllo delle infezioni e sicurezza del paziente

Risposta: C · On clean, dry skin over a well-perfused muscle, close to the surgical site

Perché è questa la risposta

The pad returns the current safely. Good contact over a well-perfused muscle prevents burns, while bony areas, scars, hair and moisture reduce contact and can cause burns at the pad site.

Risposta

C · On clean, dry skin over a well-perfused muscle, close to the surgical site
4.Controllo delle infezioni e sicurezza del paziente

Risposta: C · Clarify the reaction, update the allergy record and tell the prescriber

Perché è questa la risposta

Bedside handoff lets patients correct errors. The nurse asks about the reaction, updates the record and alerts the prescriber and pharmacy so current and future orders can be checked.

Risposta

C · Clarify the reaction, update the allergy record and tell the prescriber
5.Valori di laboratorio ed elettroliti

Risposta: A · Serum sodium, which may fall too low

Perché è questa la risposta

Desmopressin acts like ADH, so the kidneys retain water. If the patient drinks too much, water retention can cause hyponatremia, which may lead to confusion and seizures. Sodium, weight and fluid balance are monitored.

Risposta

A · Serum sodium, which may fall too low

7.Infermieristica clinica

Risposta: C · Breathing function, such as forced vital capacity

Perché è questa la risposta

Guillain–Barré syndrome can paralyse the respiratory muscles. Serial vital capacity measurements help identify patients who need ventilation before they deteriorate.

Risposta

C · Breathing function, such as forced vital capacity
8.Infermieristica clinica

Risposta: B · The cold protects the brain, so recovery may be possible after long resuscitation

Perché è questa la risposta

Hypothermia reduces the brain's oxygen needs, and good recoveries have occurred after long resuscitation. Death is usually not confirmed until the patient has been rewarmed, unless injuries are clearly not survivable.

Risposta

B · The cold protects the brain, so recovery may be possible after long resuscitation
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: B · Left ventricular hypertrophy on the ECG

Perché è questa la risposta

Target organ damage from hypertension includes left ventricular hypertrophy, retinopathy, chronic kidney disease with proteinuria, and stroke. Its presence changes treatment urgency and targets.

Risposta

B · Left ventricular hypertrophy on the ECG
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · Compartment syndrome from reperfusion swelling

Perché è questa la risposta

When blood flow returns to ischaemic muscle, swelling can raise pressure in a fascial compartment. Pain out of proportion and pain on passive stretch are early signs; pulses may remain until late. It is reported urgently, as fasciotomy may be needed. The limb is kept at heart level, not raised high.

Risposta

C · Compartment syndrome from reperfusion swelling
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta

D · In un apposito scaldatore a calore secco
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: D · Obstructive sleep apnoea

Perché è questa la risposta

Growth hormone excess enlarges the tongue and upper airway tissues, so obstructive sleep apnoea is common in acromegaly. It needs assessment because it raises cardiovascular risk and affects safety under anaesthesia.

Risposta

D · Obstructive sleep apnoea
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: B · Treatment before the main treatment, often to shrink the tumour

Perché è questa la risposta

Neoadjuvant chemotherapy or radiotherapy is given before surgery to shrink the tumour, which may make surgery easier or allow less extensive surgery. Adjuvant therapy follows surgery.

Risposta

B · Treatment before the main treatment, often to shrink the tumour
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: B · 500 mL/h

Perché è questa la risposta

Parkland gives 4 × 60 × 25 = 6000 mL in 24 hours, with 3000 mL due in the first 8 hours from the time of injury. With 6 hours left, 3000 ÷ 6 = 500 mL/h. Dividing by 8 (375 mL/h) ignores the delay. The rate is then adjusted to urine output.

Risposta

B · 500 mL/h
