1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: B · Write the order down and read it back to the provider to confirm it

Perché è questa la risposta

For telephone and verbal orders, the nurse writes down the full order and reads it back so the provider can confirm it. This “read-back” prevents errors from mishearing.

Risposta

B · Write the order down and read it back to the provider to confirm it
2.Priorità e delega

Risposta: C · Standard precautions, because it does not usually spread from person to person

Perché è questa la risposta

Inhalational anthrax is not spread from person to person, so standard precautions are used. Contact precautions may be added for skin (cutaneous) anthrax with draining lesions.

Risposta

C · Standard precautions, because it does not usually spread from person to person
3.Controllo delle infezioni e sicurezza del paziente

Risposta: A · Hold the bottle with the label in the palm and pour without touching the bowl

Perché è questa la risposta

The nurse pours from a little height without touching the sterile bowl, avoiding splashes that would wet the field. Holding the label in the palm keeps it readable and free of drips.

Risposta

A · Hold the bottle with the label in the palm and pour without touching the bowl
4.Controllo delle infezioni e sicurezza del paziente

Risposta: B · Into a red outlet connected to the emergency generator

Perché è questa la risposta

In US hospitals, red outlets are usually connected to the emergency generator. Life-support equipment is plugged into them, and battery backups are checked as part of emergency planning.

Risposta

B · Into a red outlet connected to the emergency generator
5.Valori di laboratorio ed elettroliti

Risposta: D · Gently inverting the tubes instead of shaking them

Perché è questa la risposta

Hemolysis falsely raises potassium and affects other results. It is reduced by gentle inversion of tubes, avoiding very small needles, avoiding strong suction or forcing blood through a needle, and keeping tourniquet time short.

Risposta

D · Gently inverting the tubes instead of shaking them

7.Infermieristica clinica

Risposta: A · Reduced kidney function

Perché è questa la risposta

Creatinine is a muscle waste product filtered by the kidneys. A rise shows the glomerular filtration rate is falling, as in acute kidney injury.

Risposta

A · Reduced kidney function
8.Infermieristica clinica

Risposta: A · Beta-blockers can mask warning signs such as tremor and a fast heart rate

Perché è questa la risposta

Tremor, palpitations and anxiety in hypoglycaemia are driven by the sympathetic system, which beta-blockers blunt. Sweating is often still present, and patients need to monitor glucose closely.

Risposta

A · Beta-blockers can mask warning signs such as tremor and a fast heart rate
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta

D · Vaccinazione annuale contro l'influenza e contro lo pneumococco
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · Directly observed therapy with support for barriers to adherence

Perché è questa la risposta

Directly observed therapy, where a health worker watches each dose, plus help with practical and social barriers, improves completion and reduces multidrug-resistant TB. Single-drug therapy or interrupted treatment breeds resistance.

Risposta

C · Directly observed therapy with support for barriers to adherence
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: C · At the tragus of the ear

Perché è questa la risposta

The EVD is levelled at the tragus, which approximates the foramen of Monro. If the head of the bed is changed, the drain must be re-levelled, because its height decides how much CSF drains.

Risposta

C · At the tragus of the ear
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: B · AKI from rhabdomyolysis, treated with generous IV fluids

Perché è questa la risposta

Prolonged pressure damages muscle (rhabdomyolysis), releasing myoglobin, which is toxic to the tubules, as well as potassium. Early, generous IV fluids with close monitoring of urine output, potassium and creatinine help protect the kidneys.

Risposta

B · AKI from rhabdomyolysis, treated with generous IV fluids
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: C · Blood glucose and signs of line infection

Perché è questa la risposta

PN can cause hyperglycaemia and electrolyte disturbance, and the central line is a route for bloodstream infection. Glucose, electrolytes, fluid balance, weight and temperature are monitored closely.

Risposta

C · Blood glucose and signs of line infection
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · Perforation with peritonitis

Perché è questa la risposta

Brief relief can occur when an inflamed appendix ruptures, followed by spreading peritonitis with fever, rigidity and tachycardia. This is a surgical emergency needing urgent review, IV fluids and antibiotics.

Risposta

D · Perforation with peritonitis
