1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: A · “I will press the button for her while she is asleep so she stays comfortable.”

Perché è questa la risposta

PCA is safe because a sleepy patient cannot press the button. If someone else presses it, the patient can receive too much opioid and develop respiratory depression.

Risposta

A · “I will press the button for her while she is asleep so she stays comfortable.”
2.Priorità e delega

Risposta: C · Defibrillate as soon as the defibrillator is available, with CPR until then

Perché è questa la risposta

Ventricular fibrillation is a shockable cardiac arrest rhythm. Early defibrillation, with high-quality CPR until the shock is given, is the most important treatment.

Risposta

C · Defibrillate as soon as the defibrillator is available, with CPR until then
3.Controllo delle infezioni e sicurezza del paziente

Risposta: B · When a patient shows an acute decline, such as new confusion or a sharp change in vital signs

Perché è questa la risposta

Rapid response teams bring critical care skills to a patient who is getting worse, with the aim of preventing cardiac arrest or ICU transfer. Any nurse who is worried about an acute change can call them.

Risposta

B · When a patient shows an acute decline, such as new confusion or a sharp change in vital signs
4.Controllo delle infezioni e sicurezza del paziente

Risposta: B · “If he won’t stop crying, a quick shake will get his attention.”

Perché è questa la risposta

Shaking an infant can cause bleeding in the brain, permanent disability or death. Parents are taught to put the baby down safely and step away if they feel overwhelmed, and to ask for help.

Risposta

B · “If he won’t stop crying, a quick shake will get his attention.”
5.Valori di laboratorio ed elettroliti

Risposta: D · It falls, because GFR rises

Perché è questa la risposta

In pregnancy, kidney blood flow and GFR rise by about 50%, so creatinine falls below non-pregnant values. A creatinine that looks "normal" for a non-pregnant adult may indicate reduced kidney function in pregnancy.

Risposta

D · It falls, because GFR rises
6.Valori di laboratorio ed elettroliti

Risposta: C · IV hydration, a uric acid-lowering drug and frequent electrolytes

Perché è questa la risposta

Prevention includes aggressive IV hydration, drugs such as allopurinol or rasburicase to lower uric acid, and frequent monitoring of potassium, phosphate, calcium, uric acid and creatinine. Tumor lysis usually begins within 1–3 days of starting treatment.

Risposta

C · IV hydration, a uric acid-lowering drug and frequent electrolytes
7.Infermieristica clinica

Risposta: A · To rule out bleeding in the brain

Perché è questa la risposta

A CT scan distinguishes haemorrhagic from ischaemic stroke. Clot-busting drugs are only given when bleeding has been excluded and other criteria are met.

Risposta

A · To rule out bleeding in the brain
8.Infermieristica clinica

Risposta: B · Strict hand hygiene by everyone who has contact with the patient

Perché è questa la risposta

Hand hygiene is the single most effective measure to protect neutropenic patients. Rectal procedures and IM injections are avoided because they can cause mucosal or tissue injury and bleeding.

Risposta

B · Strict hand hygiene by everyone who has contact with the patient
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: D · 80 mmHg

Perché è questa la risposta

MAP ≈ diastolic + (systolic − diastolic) ÷ 3 = 60 + 60 ÷ 3 = 80 mmHg. The heart spends longer in diastole, so MAP is closer to the diastolic value. A MAP of at least about 65 mmHg is a common target for organ perfusion.

Risposta

D · 80 mmHg
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · Cover it with a vented chest seal or a dressing taped on three sides

Perché è questa la risposta

An open (sucking) chest wound lets air enter the pleural space. A vented chest seal or a dressing taped on three sides acts as a flap valve, letting air out but not in. A fully sealed dressing can cause a tension pneumothorax. A chest drain is placed away from the wound.

Risposta

C · Cover it with a vented chest seal or a dressing taped on three sides
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta

D · In un apposito scaldatore a calore secco
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: B · Post-dural puncture headache

Perché è questa la risposta

A positional headache after a lumbar puncture, without fever or neck stiffness, fits a CSF leak. It is managed with rest, fluids, analgesia and caffeine, and an epidural blood patch if it persists. Fever or neck stiffness would suggest infection instead.

Risposta

B · Post-dural puncture headache
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: B · Making sure they have their glasses and hearing aids

Perché è questa la risposta

Delirium prevention includes using glasses and hearing aids, orientation, good sleep, hydration, nutrition, early mobilisation, pain control, avoiding unnecessary sedatives and involving family.

Risposta

B · Making sure they have their glasses and hearing aids
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: B · Treat it as a possible burn, report it and complete an incident form

Perché è questa la risposta

A blister under the return electrode suggests a diathermy burn. The nurse assesses and dresses it, informs the team, and reports it as an incident so that the cause can be investigated.

Risposta

B · Treat it as a possible burn, report it and complete an incident form
