1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: B · Keep the head of the bed raised to at least 30 degrees

Perché è questa la risposta

Raising the head of the bed to at least 30 degrees during and after feeding lowers the risk of reflux and aspiration. Lying flat greatly increases the risk.

Risposta

B · Keep the head of the bed raised to at least 30 degrees

Risposta: D · Show them how to use the gown and gloves and how to clean their hands

Perché è questa la risposta

Visitors are taught how to put on and remove PPE as required by the policy and how to perform hand hygiene, so they protect themselves and other patients.

Risposta

D · Show them how to use the gown and gloves and how to clean their hands
4.Controllo delle infezioni e sicurezza del paziente

Risposta: A · Finding the system and process weaknesses that allowed the error

Perché è questa la risposta

A root cause analysis looks beyond the individual to the system factors, such as storage, labeling, staffing or workflow, that allowed the error. The aim is to change the system so the error cannot happen again.

Risposta

A · Finding the system and process weaknesses that allowed the error
5.Valori di laboratorio ed elettroliti

Risposta: D · A rise in serum uric acid

Perché è questa la risposta

Thiazides reduce uric acid excretion and can trigger gout. They also tend to lower potassium, sodium and magnesium, raise calcium (by reducing its excretion) and raise blood glucose and lipids.Verificato con: Hydrochlorothiazide · StatPearls (NCBI Bookshelf) (2026)

Risposta

D · A rise in serum uric acid
6.Valori di laboratorio ed elettroliti

Risposta: A · A chronic stable rise; infarction needs a rising or falling pattern

Perché è questa la risposta

Troponin is often chronically raised in CKD and other conditions. Acute infarction is diagnosed by a rise and/or fall in troponin together with symptoms or ECG changes, so serial results are compared with the patient's baseline.Verificato con: Troponin Test · MedlinePlus (NIH) (2023)

Risposta

A · A chronic stable rise; infarction needs a rising or falling pattern
7.Infermieristica clinica

Risposta: C · Potassium, because both drugs can raise it

Perché è questa la risposta

Spironolactone and ACE inhibitors both reduce potassium excretion, so the combination can cause dangerous hyperkalaemia. Potassium and renal function should be checked regularly.

Risposta

C · Potassium, because both drugs can raise it
8.Infermieristica clinica

Risposta: C · An epidural haematoma or abscess compressing the spinal cord

Perché è questa la risposta

Increasing motor block, back pain or new weakness after an epidural can mean a haematoma or abscess compressing the cord. This is a surgical emergency: stop the infusion and get urgent medical review.

Risposta

C · An epidural haematoma or abscess compressing the spinal cord
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: B · Regular sawtooth flutter waves between QRS complexes

Perché è questa la risposta

Atrial flutter is caused by a re-entry circuit in the right atrium, giving regular sawtooth flutter waves, often at about 300/min with 2:1 block (ventricular rate about 150/min). A chaotic baseline with irregular QRS is atrial fibrillation. Like AF, it carries a stroke risk.

Risposta

B · Regular sawtooth flutter waves between QRS complexes
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: D · Not start CPR, call for support, and provide dignity and comfort

Perché è questa la risposta

A valid do-not-attempt-CPR decision means CPR is not attempted. Other appropriate care continues, the senior team and family are informed and the patient is cared for with dignity. Nurses should check that the decision is valid and applies to the situation.

Risposta

D · Not start CPR, call for support, and provide dignity and comfort
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: D · To match the natural cortisol rhythm and protect the stomach

Perché è questa la risposta

Morning dosing mimics the normal early-morning cortisol peak, causes less adrenal suppression and less sleep disturbance. Taking it with food reduces gastric irritation.

Risposta

D · To match the natural cortisol rhythm and protect the stomach
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: A · A 78-year-old who takes warfarin for atrial fibrillation

Perché è questa la risposta

Anticoagulation greatly increases the risk of an intracranial bleed after even a minor head injury, and older age adds to the risk. These patients have a low threshold for a CT scan, even when they appear well.

Risposta

A · A 78-year-old who takes warfarin for atrial fibrillation
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · Serotonin syndrome with some antidepressants, and seizures

Perché è questa la risposta

Tramadol is a weak opioid that also inhibits serotonin and noradrenaline reuptake. It can lower the seizure threshold and cause serotonin syndrome with SSRIs and other serotonergic drugs.

Risposta

D · Serotonin syndrome with some antidepressants, and seizures
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: B · Explore their beliefs and explain how analgesia aids recovery

Perché è questa la risposta

Patients may under-report pain because of beliefs, fear of addiction or not wanting to bother staff. Exploring these concerns and explaining that good analgesia helps breathing and mobility supports a shared plan.

Risposta

B · Explore their beliefs and explain how analgesia aids recovery
