1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: B · Assess the lungs, oxygen saturation and swelling, then inform the provider

Perché è questa la risposta

A rapid weight gain with orthopnea suggests fluid overload and worsening heart failure. The nurse assesses breathing and fluid status and informs the provider, who may adjust the diuretic.

Risposta

B · Assess the lungs, oxygen saturation and swelling, then inform the provider
2.Priorità e delega

Risposta: A · Restlessness and a rising heart rate while the blood pressure is still normal

Perché è questa la risposta

In early shock, the body keeps the blood pressure up by raising the heart rate and narrowing vessels, so restlessness and tachycardia appear first. A falling blood pressure is a later sign.

Risposta

A · Restlessness and a rising heart rate while the blood pressure is still normal
3.Controllo delle infezioni e sicurezza del paziente

Risposta: D · In a bassinet, not carried in the arms

Perché è questa la risposta

Infants are moved in a bassinet through corridors. This prevents drops and makes abduction easier to spot, because carrying a baby in the arms in a hallway is unusual.

Risposta

D · In a bassinet, not carried in the arms
4.Controllo delle infezioni e sicurezza del paziente

Risposta: B · Pulse oximetry cannot tell carbon monoxide-bound hemoglobin from oxygen-bound hemoglobin

Perché è questa la risposta

Standard pulse oximeters read carboxyhemoglobin as oxyhemoglobin, so SpO2 can look normal in carbon monoxide poisoning. A blood carboxyhemoglobin level confirms it, and high-flow oxygen is given.

Risposta

B · Pulse oximetry cannot tell carbon monoxide-bound hemoglobin from oxygen-bound hemoglobin
5.Valori di laboratorio ed elettroliti

Risposta: D · Have the patient void and discard that urine, then save all urine until 08:00 the next day

Perché è questa la risposta

The collection starts with an empty bladder, so the first void is discarded and the time recorded. All urine for the next 24 hours is saved, including a final void at the end time. If urine is discarded by mistake, the collection usually has to restart.Verificato con: Urine 24-hour volume · MedlinePlus (NIH) (2025)

Risposta

D · Have the patient void and discard that urine, then save all urine until 08:00 the next day
6.Valori di laboratorio ed elettroliti

Risposta: D · "I will cut out salt from my diet completely to stay healthy"

Perché è questa la risposta

A sudden fall in sodium intake makes the kidneys keep more sodium and lithium, raising the lithium level towards toxicity. Patients keep a normal, steady salt intake, drink enough fluids, report fluid loss and avoid NSAIDs unless approved.

Risposta

D · "I will cut out salt from my diet completely to stay healthy"
7.Infermieristica clinica

Risposta: C · Peritonite

Risposta

C · Peritonite
8.Infermieristica clinica

Risposta: A · "I'll drink as little as possible so I don't leak."

Perché è questa la risposta

Restricting fluids concentrates urine, irritates the bladder and risks dehydration and infection. Pelvic floor exercises, weight loss and reducing caffeine are recommended measures.

Risposta

A · "I'll drink as little as possible so I don't leak."
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: B · To prevent dehydration and keep secretions thinner

Perché è questa la risposta

Fever, sweating and tachypnoea increase insensible fluid loss. Good hydration prevents dehydration and helps keep secretions less sticky. Patients with heart failure or other restrictions need individual plans.

Risposta

B · To prevent dehydration and keep secretions thinner
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: D · Tube displacement, disconnection or obstruction

Perché è questa la risposta

A sudden loss of the capnography trace means no CO₂ is being detected: the tube may be dislodged, disconnected or blocked, or there may be cardiac arrest. The nurse checks the patient and circuit immediately, hand-ventilates if needed and calls for help.

Risposta

D · Tube displacement, disconnection or obstruction
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: C · Drink plenty of fluid so the urine stays pale

Perché è questa la risposta

High fluid intake is the most important measure for preventing stones. Dietary calcium is usually not restricted, because it binds oxalate in the gut. High-dose vitamin C can increase oxalate.

Risposta

C · Drink plenty of fluid so the urine stays pale
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: D · "If she is unconscious, I will pour juice into her mouth."

Perché è questa la risposta

An unconscious person cannot swallow safely, so fluids in the mouth can be aspirated. The family should put the person in the recovery position, give glucagon if trained and call for help. Once awake and able to swallow, the person eats a longer-acting carbohydrate.

Risposta

D · "If she is unconscious, I will pour juice into her mouth."
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · Breathe in slowly and deeply, then hold briefly

Perché è questa la risposta

Slow, deep inspiration with a short hold expands the alveoli and helps prevent atelectasis. It is repeated regularly while awake, together with coughing and good pain control.

Risposta

D · Breathe in slowly and deeply, then hold briefly
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta

C · Un ematoma epidurale, che richiede valutazione d'emergenza e RM
