1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: A · “I will keep my spare insulin in the freezer so it lasts longer.”

Perché è questa la risposta

Freezing damages insulin, and frozen insulin should not be used. Unopened insulin is kept in the fridge, and heat and expired products are avoided.

Risposta

A · “I will keep my spare insulin in the freezer so it lasts longer.”
2.Priorità e delega

Risposta: A · Level 2

Perché è questa la risposta

Active suicidal ideation with a plan is a high-risk situation, so the patient is ESI level 2. They need prompt evaluation and a safe environment with close observation.

Risposta

A · Level 2
3.Controllo delle infezioni e sicurezza del paziente

Risposta: D · Remove the catheter and insert a new one at another site if still needed

Perché è questa la risposta

Redness, warmth and tenderness along the vein indicate phlebitis, which can lead to infection. The catheter is removed and, if IV access is still needed, a new one is placed at a different site.

Risposta

D · Remove the catheter and insert a new one at another site if still needed
4.Controllo delle infezioni e sicurezza del paziente

Risposta: A · The second nurse checks the order, drug, dose and pump settings separately without being told the answer

Perché è questa la risposta

In an independent double check, the second person works through each step on their own, then the two results are compared. Reading the answer aloud or glancing invites confirmation bias.

Risposta

A · The second nurse checks the order, drug, dose and pump settings separately without being told the answer
5.Valori di laboratorio ed elettroliti

Risposta: C · Serum calcium

Perché è questa la risposta

In severe pancreatitis, fatty acids released from damaged fat bind calcium (saponification), causing hypocalcemia. Twitching and perioral numbness are signs of low calcium, which can progress to tetany and seizures.Verificato con: Calcium Blood Test · MedlinePlus (NIH) (2024)

Risposta

C · Serum calcium
6.Valori di laboratorio ed elettroliti

Risposta: A · IV thiamine

Perché è questa la risposta

People with alcohol use disorder and malnutrition are often deficient in thiamine. Giving glucose first can use up the remaining thiamine and precipitate Wernicke encephalopathy, so thiamine is usually given before or with glucose. Hypoglycemia must still be treated promptly.

Risposta

A · IV thiamine
7.Infermieristica clinica

Risposta: D · Their stools may turn dark or black

Perché è questa la risposta

Oral iron commonly darkens the stool and can cause constipation or stomach upset. Tea and antacids reduce absorption, while vitamin C may increase it.

Risposta

D · Their stools may turn dark or black
8.Infermieristica clinica

Risposta: D · Cardiac tamponade

Perché è questa la risposta

Hypotension, raised JVP and muffled heart sounds (Beck's triad) suggest cardiac tamponade. Blood in the pericardial space compresses the heart and needs urgent drainage.

Risposta

D · Cardiac tamponade
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: B · Rapid high doses can damage hearing

Perché è questa la risposta

High plasma levels of loop diuretics can cause tinnitus and hearing loss (ototoxicity), especially with fast injection, renal impairment or other ototoxic drugs such as aminoglycosides. Large IV doses are therefore given slowly.

Risposta

B · Rapid high doses can damage hearing
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: D · Systemic thrombolysis

Perché è questa la risposta

A PE causing sustained hypotension is high-risk and usually treated with thrombolysis to dissolve the clot quickly, unless contraindicated, when surgical or catheter removal may be used. Stable patients are usually treated with anticoagulation alone.

Risposta

D · Systemic thrombolysis
11.Area medico-chirurgica: neurologica, endocrina e renale

12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: A · Loss of the outer half of the field in both eyes

Perché è questa la risposta

The chiasm carries crossing fibres from the nasal retinas, which see the temporal (outer) fields. Compression causes a bitemporal hemianopia. Patients may bump into objects at their sides.

Risposta

A · Loss of the outer half of the field in both eyes
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: C · An urgent review by a clinician skilled in acute illness

Perché è questa la risposta

NEWS2 scores respiratory rate, SpO₂, oxygen use, BP, pulse, consciousness or new confusion, and temperature. A score of 5–6 needs an urgent response, and 7 or more an emergency response.

Risposta

C · An urgent review by a clinician skilled in acute illness
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · Liver damage may still develop, and appears only days later

Perché è questa la risposta

Early after paracetamol overdose, patients often have no or few symptoms, but liver injury peaks after about 3–4 days. Treatment with acetylcysteine is guided by timed paracetamol levels and the history.

Risposta

D · Liver damage may still develop, and appears only days later
