1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: B · “My husband can smoke in my room if the window is open.”

Perché è questa la risposta

Oxygen supports combustion, so no one may smoke near it, even with a window open. Keeping it away from flames and avoiding oil-based products are correct.

Risposta

B · “My husband can smoke in my room if the window is open.”
2.Priorità e delega

Risposta: B · Bleeding from the operation site

Perché è questa la risposta

Frequent swallowing after tonsillectomy is a classic sign of bleeding. The nurse checks the throat with a light, looks at vital signs and reports at once.

Risposta

B · Bleeding from the operation site
3.Controllo delle infezioni e sicurezza del paziente

Risposta: B · The surface stays wet for the contact time on the product label

Perché è questa la risposta

Each disinfectant has a contact (dwell) time during which the surface must stay visibly wet. Drying it early or spreading one wipe over many items reduces its effect.

Risposta

B · The surface stays wet for the contact time on the product label
4.Controllo delle infezioni e sicurezza del paziente

Risposta: D · Removing the patient’s clothing

Perché è questa la risposta

Removing clothing, then gently washing the skin, gets rid of most external radioactive contamination. Hard scrubbing damages the skin, and potassium iodide only protects the thyroid from radioactive iodine.

Risposta

D · Removing the patient’s clothing
5.Valori di laboratorio ed elettroliti

Risposta: A · The NSAID reduces kidney blood flow on top of the other two drugs

Perché è questa la risposta

NSAIDs reduce prostaglandin-driven blood flow to the kidney. Combined with an ACE inhibitor and a diuretic, this can cause acute kidney injury, especially in older adults. The nurse reports the result and the NSAID is usually stopped.Verificato con: Creatinine blood test · MedlinePlus (NIH) (2025)

Risposta

A · The NSAID reduces kidney blood flow on top of the other two drugs
6.Valori di laboratorio ed elettroliti

Risposta: B · A very high creatine kinase (CK)

Perché è questa la risposta

These signs suggest neuroleptic malignant syndrome, a medical emergency. Muscle rigidity releases CK, often to very high levels, and the white count is often raised. Myoglobin can damage the kidneys. The antipsychotic is stopped and supportive care is started.

Risposta

B · A very high creatine kinase (CK)
7.Infermieristica clinica

Risposta: B · Repositioning and distraction

Perché è questa la risposta

Positioning, heat or cold where safe, relaxation, distraction and explanation can all reduce pain alongside analgesics.

Risposta

B · Repositioning and distraction
8.Infermieristica clinica

Risposta: B · A catheter-related bloodstream infection

Perché è questa la risposta

Rigors after flushing suggest bacteria colonising the catheter. Blood cultures are taken (often from the line and a peripheral vein) and the team decides whether to remove the line.

Risposta

B · A catheter-related bloodstream infection
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · Pneumonia or cancer affecting the pleura

Perché è questa la risposta

Transudates form when hydrostatic or oncotic pressure changes push fluid out of vessels, as in heart failure, cirrhosis and nephrotic syndrome. Exudates form when inflammation or tumour makes the pleura leaky, so protein and LDH are high (Light's criteria). Common causes are pneumonia, cancer, TB and pulmonary embolism.

Risposta

C · Pneumonia or cancer affecting the pleura
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: A · The right main bronchus

Perché è questa la risposta

Inhaled foreign bodies most often enter the right main bronchus and right lower lobe, because the right bronchus is wider, shorter and more vertical. Signs include a unilateral wheeze, reduced breath sounds and recurrent infection. Bronchoscopy is used for removal.

Risposta

A · The right main bronchus
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta

C · Iperaldosteronismo primario (di Conn)
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta

B · Monitoraggio attento di diuresi, sodio e osmolalità
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: A · A raised level suggests poor tissue perfusion

Perché è questa la risposta

A high lactate signals tissue hypoperfusion and higher risk of death in sepsis. It helps guide fluid resuscitation and escalation, and it is repeated to check the response.

Risposta

A · A raised level suggests poor tissue perfusion
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: C · Cardiac tamponade from a pericardial effusion

Perché è questa la risposta

Malignant pericardial effusion can compress the heart, causing hypotension, raised JVP and muffled heart sounds. It is an emergency needing urgent echocardiography and drainage (pericardiocentesis).

Risposta

C · Cardiac tamponade from a pericardial effusion
