1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: C · Diphenhydramine

Perché è questa la risposta

Diphenhydramine is strongly anticholinergic and can cause confusion, urinary retention and falls in older adults. It is on lists of drugs to avoid in this group.

Risposta

C · Diphenhydramine
2.Priorità e delega

Risposta: A · “I can still eat aged cheese and cured salami.”

Perché è questa la risposta

MAO inhibitors with tyramine-rich foods, such as aged cheese and cured meats, can cause a hypertensive crisis. Patients avoid these foods, check medicines with a pharmacist and report a severe headache.

Risposta

A · “I can still eat aged cheese and cured salami.”
3.Controllo delle infezioni e sicurezza del paziente

Risposta: D · Separate from other drugs, with a warning that they cause paralysis

Perché è questa la risposta

Neuromuscular blockers stop breathing. They are stored apart from other drugs, often in sealed or colored containers with warnings such as “Paralyzing agent: causes respiratory arrest”.

Risposta

D · Separate from other drugs, with a warning that they cause paralysis
4.Controllo delle infezioni e sicurezza del paziente

Risposta: B · Draw the blood cultures first, then give the antibiotic without delay

Perché è questa la risposta

Cultures are drawn before the first antibiotic dose so the organism can still be grown and identified. The antibiotic is then given promptly, because delay increases the risk of death in sepsis.

Risposta

B · Draw the blood cultures first, then give the antibiotic without delay
5.Valori di laboratorio ed elettroliti

Risposta: C · Low TSH with a high free T4

Perché è questa la risposta

In Graves disease, antibodies stimulate the thyroid to make too much hormone. The high free T4 suppresses TSH from the pituitary through negative feedback, so TSH is low. A high TSH with a low T4 fits primary hypothyroidism.Verificato con: TSH test · MedlinePlus (NIH) (2026); Thyroxine (T4) Test · MedlinePlus (NIH) (2024); Graves disease · MedlinePlus (NIH) (2026)

Risposta

C · Low TSH with a high free T4
6.Valori di laboratorio ed elettroliti

Risposta: C · Primary hyperparathyroidism

Perché è questa la risposta

In primary hyperparathyroidism, usually from an adenoma, PTH is high despite high calcium, and phosphate is low because PTH increases its excretion. Cancer-related hypercalcemia typically has a suppressed PTH.Verificato con: Parathyroid Hormone (PTH) Test · MedlinePlus (NIH) (2023); Hyperparathyroidism · MedlinePlus (NIH) (2026)

Risposta

C · Primary hyperparathyroidism
7.Infermieristica clinica

Risposta: A · To detect diabetic retinopathy early, before sight is lost

Perché è questa la risposta

Diabetic retinopathy often has no symptoms until it is advanced. Screening allows early treatment that can prevent blindness.

Risposta

A · To detect diabetic retinopathy early, before sight is lost
8.Infermieristica clinica

Risposta: D · Review and stop treatments that no longer help, and focus on comfort

Perché è questa la risposta

When dying is recognised, non-beneficial interventions are stopped and anticipatory medicines for symptoms such as pain, agitation and secretions are prescribed. Clear communication with the patient and family is essential.

Risposta

D · Review and stop treatments that no longer help, and focus on comfort
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: D · It can briefly worsen symptoms before long-term benefit appears

Perché è questa la risposta

Beta-blockers improve survival in heart failure with reduced ejection fraction, but they can cause tiredness, low blood pressure, bradycardia and fluid retention when first started. They are therefore started low and increased gradually in stable patients, with monitoring of weight, pulse and BP.

Risposta

D · It can briefly worsen symptoms before long-term benefit appears
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: B · Respect the decision, inform the prescriber and discuss non-animal options

Perché è questa la risposta

Most heparins are derived from porcine intestine. Patients have the right to know and to decline. The nurse informs the prescriber, who can consider synthetic alternatives (such as fondaparinux) or mechanical prophylaxis, and documents the discussion.

Risposta

B · Respect the decision, inform the prescriber and discuss non-animal options
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: B · Ask questions that can be answered with yes, no or a gesture

Perché è questa la risposta

In expressive aphasia, comprehension is largely preserved. Short, closed questions, extra time, gestures and picture or word boards help. Shouting does not help, and constantly finishing sentences can frustrate the patient.

Risposta

B · Ask questions that can be answered with yes, no or a gesture
12.Area medico-chirurgica: neurologica, endocrina e renale

13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: A · Stopping smoking

Perché è questa la risposta

Smoking worsens Crohn's disease, increasing flares, complications and the need for surgery. Stopping smoking is one of the most effective changes a patient can make.

Risposta

A · Stopping smoking
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: C · Cancer and its treatment greatly increase clot risk

Perché è questa la risposta

Cancer, chemotherapy, central lines and immobility increase the risk of DVT and PE. Unilateral leg swelling needs assessment with ultrasound and possible anticoagulation.

Risposta

C · Cancer and its treatment greatly increase clot risk
