1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: B · “I would like you to see him within 15 minutes and consider a fluid bolus.”

Perché è questa la risposta

A good recommendation is specific about the request and the time frame. Vague statements leave the provider unsure how urgent the problem is.

Risposta

B · “I would like you to see him within 15 minutes and consider a fluid bolus.”
2.Priorità e delega

Risposta: A · Nero (deceduto o atteso)

Risposta

A · Nero (deceduto o atteso)
3.Controllo delle infezioni e sicurezza del paziente

Risposta: B · In a bassinet, not carried in the arms

Perché è questa la risposta

Infants are moved in a bassinet through corridors. This prevents drops and makes abduction easier to spot, because carrying a baby in the arms in a hallway is unusual.

Risposta

B · In a bassinet, not carried in the arms
4.Controllo delle infezioni e sicurezza del paziente

Risposta: B · Finding the system and process weaknesses that allowed the error

Perché è questa la risposta

A root cause analysis looks beyond the individual to the system factors, such as storage, labeling, staffing or workflow, that allowed the error. The aim is to change the system so the error cannot happen again.

Risposta

B · Finding the system and process weaknesses that allowed the error
5.Valori di laboratorio ed elettroliti

Risposta: D · To avoid diluting the sample with flush or infusion fluid

Perché è questa la risposta

Central lines contain flush solution or infused fluids, which can dilute the sample or add glucose, heparin or electrolytes. Infusions are paused and an initial volume is discarded (or reinfused per policy) before the sample is drawn; then the line is flushed.

Risposta

D · To avoid diluting the sample with flush or infusion fluid

7.Infermieristica clinica

Risposta: A · Avoid large meals late in the evening and raise the head of the bed

Perché è questa la risposta

Smaller meals, not eating close to bedtime, raising the head of the bed, weight loss and stopping smoking reduce reflux. Lying flat after eating makes it worse.

Risposta

A · Avoid large meals late in the evening and raise the head of the bed
8.Infermieristica clinica

Risposta: B · Spasm of the hand when a blood pressure cuff is inflated on the arm

Perché è questa la risposta

Trousseau's sign, carpal spasm during cuff inflation, and Chvostek's sign, facial twitching on tapping the facial nerve, indicate neuromuscular irritability from low calcium.

Risposta

B · Spasm of the hand when a blood pressure cuff is inflated on the arm
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · It suggests tissues are short of oxygen and using anaerobic metabolism

Perché è questa la risposta

When tissues are poorly perfused, cells switch to anaerobic metabolism and produce lactate. A raised lactate is a marker of tissue hypoperfusion and severity in sepsis and shock. Repeated measurements show whether treatment is working.

Risposta

C · It suggests tissues are short of oxygen and using anaerobic metabolism
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: A · Slow to 100–120/min and allow full chest recoil

Perché è questa la risposta

High-quality adult CPR uses a rate of 100–120/min, a depth of 5–6 cm, full recoil after each compression and minimal interruptions. Leaning prevents the chest from re-expanding, so the heart fills less. Compressors should swap about every 2 minutes to avoid fatigue.

Risposta

A · Slow to 100–120/min and allow full chest recoil
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: C · Receptive (Wernicke's) aphasia

Perché è questa la risposta

Receptive aphasia comes from damage to Wernicke's area in the dominant temporal lobe. Speech is fluent but often meaningless, and comprehension is impaired. Patients are often unaware of their errors.

Risposta

C · Receptive (Wernicke's) aphasia
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: D · IV glucose or IM glucagon, then recheck the glucose

Perché è questa la risposta

Severe hypoglycaemia can cause seizures. A patient who cannot swallow needs IV glucose, or IM glucagon if there is no IV access. Nothing should be put in the mouth of a fitting or unconscious patient. Glucose is rechecked and a long-acting carbohydrate is given when safe.

Risposta

D · IV glucose or IM glucagon, then recheck the glucose
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: B · Long inflation can cause nerve and muscle damage

Perché è questa la risposta

A tourniquet creates a bloodless field but causes limb ischaemia. Inflation time and pressure are recorded and the surgeon is told at set intervals, as long inflation can damage nerves and muscle.

Risposta

B · Long inflation can cause nerve and muscle damage
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: A · Osteomyelitis

Perché è questa la risposta

A positive probe-to-bone test in a diabetic foot ulcer strongly suggests osteomyelitis. Imaging, bone samples, prolonged antibiotics and specialist foot team care are usually needed.

Risposta

A · Osteomyelitis
