1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: A · Check the tubing for kinks or obstruction

Perché è questa la risposta

Sudden lack of drainage with a full bladder usually means a kinked or blocked catheter. The nurse checks the tubing and position first, then follows the policy for irrigation or replacement.

Risposta

A · Check the tubing for kinks or obstruction
2.Priorità e delega

Risposta: C · Sudden calf pain and swelling in the operated leg, with new breathlessness

Perché è questa la risposta

Calf pain and swelling suggest deep vein thrombosis, and new breathlessness raises concern for a pulmonary embolism. This needs immediate assessment and escalation; the other findings are expected.

Risposta

C · Sudden calf pain and swelling in the operated leg, with new breathlessness
3.Controllo delle infezioni e sicurezza del paziente

Risposta: A · The patient’s identity is checked against the blood product and its paperwork at the bedside

Perché è questa la risposta

Wrong-patient transfusion can cause a fatal ABO reaction, so the final check is always done at the bedside, with positive patient identification against the product label, compatibility tag and order. Many US hospitals require two qualified people for this check, while UK guidance allows one trained person if positive identification is used. Local policy decides who checks.

Risposta

A · The patient’s identity is checked against the blood product and its paperwork at the bedside
4.Controllo delle infezioni e sicurezza del paziente

Risposta: B · “I keep small rugs in the hall so my feet stay warm.”

Perché è questa la risposta

Loose rugs are a common cause of trips and falls and should be removed or secured. Night lights, grab bars and non-slip footwear all reduce fall risk.

Risposta

B · “I keep small rugs in the hall so my feet stay warm.”
5.Valori di laboratorio ed elettroliti

Risposta: C · Vitamin K (phytonadione)

Perché è questa la risposta

Warfarin blocks vitamin K–dependent clotting factors, and vitamin K restores their production over hours. For serious bleeding, a prothrombin complex concentrate may also be given to replace factors at once. The warfarin is held.Verificato con: Label: WARFARIN SODIUM tablet · DailyMed (NLM/FDA) (2012)

Risposta

C · Vitamin K (phytonadione)
6.Valori di laboratorio ed elettroliti

Risposta: C · Low cortisol with a high ACTH

Perché è questa la risposta

In primary adrenal insufficiency the adrenal glands fail, so cortisol is low and the pituitary responds with high ACTH, which also darkens the skin. Low cortisol with low ACTH suggests a pituitary cause. An ACTH stimulation test confirms the diagnosis.Verificato con: Adrenocorticotropic Hormone (ACTH) · MedlinePlus (NIH) (2023); Cortisol Test · MedlinePlus (NIH) (2024)

Risposta

C · Low cortisol with a high ACTH
7.Infermieristica clinica

Risposta: A · A combination of antibiotics with a proton pump inhibitor

Perché è questa la risposta

Eradication therapy combines a PPI with two antibiotics for a set course. Completing the course is important for success.

Risposta

A · A combination of antibiotics with a proton pump inhibitor
8.Infermieristica clinica

Risposta: A · They may have no fever and present with confusion, falls or reduced mobility

Perché è questa la risposta

Older people may have a normal or low temperature and present with delirium, falls or general decline. Early warning scores and a high index of suspicion are important.

Risposta

A · They may have no fever and present with confusion, falls or reduced mobility
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · Bleeding, including a sudden change in neurological status

Perché è questa la risposta

The main risk of thrombolysis is bleeding, from puncture sites, gums or the gut, and most seriously intracranial haemorrhage. The nurse monitors puncture sites, vital signs and neurological status and minimises invasive procedures. Reperfusion arrhythmias are also common.

Risposta

C · Bleeding, including a sudden change in neurological status
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · Higher chest pressure reduces venous return to the heart

Perché è questa la risposta

PEEP keeps alveoli open and improves oxygenation, but higher intrathoracic pressure impedes venous return, reducing preload and cardiac output, especially in hypovolaemic patients. The team may give fluid or adjust PEEP. High pressures also risk barotrauma.

Risposta

C · Higher chest pressure reduces venous return to the heart
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: B · Stand up slowly, because it can make you dizzy

Perché è questa la risposta

Tamsulosin relaxes smooth muscle in the prostate and bladder neck, improving urine flow within days. It can cause postural hypotension and dizziness, especially after the first dose, so patients rise slowly.

Risposta

B · Stand up slowly, because it can make you dizzy
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: D · Use healthy sites and check glucose often, as less insulin may be needed

Perché è questa la risposta

Lipohypertrophy causes poor and unpredictable absorption. When the patient switches to healthy tissue, more insulin is absorbed, so hypos can occur. Glucose is monitored closely, and doses may need to be reduced.

Risposta

D · Use healthy sites and check glucose often, as less insulin may be needed
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta

A · Punteggio di Glasgow-Blatchford
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica
