1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: C · Massage the fundus

Perché è questa la risposta

Uterine atony is the most common cause of postpartum hemorrhage. Fundal massage helps the uterus contract; the nurse then calls for help and gives prescribed uterotonics if bleeding continues.

Risposta

C · Massage the fundus
2.Priorità e delega

Risposta: C · Turn her onto her left side

Perché è questa la risposta

Lying on the back lets the uterus compress the inferior vena cava, reducing blood return to the heart. Turning to the left side relieves the pressure and restores the blood pressure.

Risposta

C · Turn her onto her left side
3.Controllo delle infezioni e sicurezza del paziente

Risposta: A · Separate from other drugs, with a warning that they cause paralysis

Perché è questa la risposta

Neuromuscular blockers stop breathing. They are stored apart from other drugs, often in sealed or colored containers with warnings such as “Paralyzing agent: causes respiratory arrest”.

Risposta

A · Separate from other drugs, with a warning that they cause paralysis
4.Controllo delle infezioni e sicurezza del paziente

Risposta: B · Yellow (delayed)

Perché è questa la risposta

The casualty cannot walk, so is not green. Breathing is under 30 a minute, perfusion is adequate and they obey commands, so none of the red criteria apply. This makes them yellow, delayed.

Risposta

B · Yellow (delayed)
5.Valori di laboratorio ed elettroliti

Risposta: C · The inflammation is settling with treatment

Perché è questa la risposta

CRP has a short half-life of about 19 hours, so it falls within days when inflammation is controlled. A falling CRP, together with clinical improvement, supports a response to treatment; a CRP that stays high or rises again needs review.Verificato con: C-reactive protein · MedlinePlus (NIH) (2025)

Risposta

C · The inflammation is settling with treatment
6.Valori di laboratorio ed elettroliti

Risposta: D · Magnesium

Perché è questa la risposta

Low magnesium reduces parathyroid hormone secretion and the body's response to it, so hypocalcemia does not correct until magnesium is replaced. The same applies to refractory hypokalemia.Verificato con: Magnesium Blood Test · MedlinePlus (NIH) (2023)

Risposta

D · Magnesium
7.Infermieristica clinica

Risposta: C · Take it on an empty stomach, usually before breakfast

Perché è questa la risposta

Levothyroxine is absorbed best on an empty stomach and is usually taken at the same time each morning. It is normally lifelong, with doses adjusted by thyroid blood tests.

Risposta

C · Take it on an empty stomach, usually before breakfast
8.Infermieristica clinica

Risposta: A · Pressione venosa giugulare aumentata e crepitii polmonari

Perché è questa la risposta

Entrambi i tipi causano tachicardia, ipotensione, cute fredda e diuresi ridotta.

Risposta

A · Pressione venosa giugulare aumentata e crepitii polmonari
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: B · It can still work, because oxygen collects in the nasopharynx

Perché è questa la risposta

Studies show that mouth breathing still draws in oxygen from the nasopharynx, so nasal cannulae can work. The effect is judged by the SpO₂, and the device is changed if targets are not reached. Cannulae are never placed in the mouth.

Risposta

B · It can still work, because oxygen collects in the nasopharynx
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: D · Systemic thrombolysis

Perché è questa la risposta

A PE causing sustained hypotension is high-risk and usually treated with thrombolysis to dissolve the clot quickly, unless contraindicated, when surgical or catheter removal may be used. Stable patients are usually treated with anticoagulation alone.

Risposta

D · Systemic thrombolysis
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: C · Acute pancreatitis

Perché è questa la risposta

GLP-1 receptor agonists have been linked to acute pancreatitis. Severe persistent abdominal pain radiating to the back needs urgent assessment, and the drug is stopped if pancreatitis is suspected.

Risposta

C · Acute pancreatitis
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: A · Escalate urgently for steroid by injection

Perché è questa la risposta

Patients on long-term steroids depend on them. If tablets cannot be absorbed, they can develop an adrenal crisis. Parenteral hydrocortisone is needed, following sick-day guidance.

Risposta

A · Escalate urgently for steroid by injection
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · Left lower abdominal pain, fever and a raised CRP

Perché è questa la risposta

Diverticulitis usually causes left iliac fossa pain and tenderness with fever, raised inflammatory markers and a change in bowel habit. CT confirms the diagnosis and shows complications such as abscess.

Risposta

D · Left lower abdominal pain, fever and a raised CRP
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · Recording baseline vital signs

Perché è questa la risposta

Trained assistants can record routine observations and report them. Assessment, teaching and interpreting results need registered nurse knowledge and judgement.

Risposta

D · Recording baseline vital signs
