1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: D · Report it, because a different oxygen device is likely to be needed

Perché è questa la risposta

Standard nasal cannulas are usually run at up to about 6 L/min; higher flows dry the nose without much benefit. If the patient still needs more oxygen, the nurse assesses and escalates for a different device.

Risposta

D · Report it, because a different oxygen device is likely to be needed
2.Priorità e delega

Risposta: D · 9

Perché è questa la risposta

Eyes open to pain scores 2, inappropriate words score 3 and withdrawal from pain scores 4, giving 9. A score of 8 or less often means the patient cannot protect their airway.

Risposta

D · 9
3.Controllo delle infezioni e sicurezza del paziente

Risposta: A · The surface stays wet for the contact time on the product label

Perché è questa la risposta

Each disinfectant has a contact (dwell) time during which the surface must stay visibly wet. Drying it early or spreading one wipe over many items reduces its effect.

Risposta

A · The surface stays wet for the contact time on the product label
4.Controllo delle infezioni e sicurezza del paziente

Risposta: D · Tell the surgeon and team, search for the item, and image the patient per policy if it is not found

Perché è questa la risposta

A count discrepancy is reported to the whole team at once. Closure waits while the field, wound and room are searched, and an x-ray is taken per policy if the item is still missing.

Risposta

D · Tell the surgeon and team, search for the item, and image the patient per policy if it is not found
5.Valori di laboratorio ed elettroliti

Risposta: C · The result may be falsely high and should be repeated with better technique

Perché è questa la risposta

Repeated fist clenching and a long tourniquet time release potassium from muscle and can cause hemolysis, both of which give a falsely high potassium. The patient is assessed and the test is repeated with correct technique; true hyperkalemia must not be missed.Verificato con: Potassium Blood Test · MedlinePlus (NIH) (2024); Potassium test · MedlinePlus (NIH) (2025)

Risposta

C · The result may be falsely high and should be repeated with better technique
6.Valori di laboratorio ed elettroliti

Risposta: D · Look for a source of blood loss in the gut

Perché è questa la risposta

Iron deficiency in an older man or a postmenopausal woman suggests chronic blood loss, often from the gastrointestinal tract, including colorectal cancer. Iron is started, but endoscopy or colonoscopy is needed to find the cause.

Risposta

D · Look for a source of blood loss in the gut
7.Infermieristica clinica

Risposta: B · Night-time hypoglycaemia

Perché è questa la risposta

Nocturnal hypoglycaemia can cause sweating, restless sleep, nightmares and morning headache. Checking glucose overnight and reviewing insulin doses can confirm and correct it.

Risposta

B · Night-time hypoglycaemia
8.Infermieristica clinica

Risposta

A · Dispnea improvvisa e dolore toracico pleuritico con tachicardia
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: A · Theophylline toxicity

Perché è questa la risposta

Theophylline has a narrow therapeutic window. Toxicity causes nausea, vomiting, tachycardia, arrhythmias, agitation and seizures. Levels are monitored, and many drugs and smoking status change its clearance. The nurse should hold the dose and report.

Risposta

A · Theophylline toxicity
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: B · Syndrome of inappropriate ADH secretion

Perché è questa la risposta

Small-cell lung cancer can secrete ADH, causing water retention and dilutional hyponatraemia (normal sodium about 135–145 mmol/L). Urine is inappropriately concentrated and the patient is not dry. Fluid restriction and treating the cancer are the main measures; sodium is corrected slowly to avoid osmotic demyelination.

Risposta

B · Syndrome of inappropriate ADH secretion
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: D · At a low dose, increased slowly

Perché è questa la risposta

Levothyroxine raises metabolic rate and myocardial oxygen demand. In older people and those with heart disease, a low starting dose that is increased gradually reduces the risk of angina and arrhythmias.

Risposta

D · At a low dose, increased slowly
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: B · Urgent drainage of the kidney plus IV antibiotics

Perché è questa la risposta

Infection behind an obstruction can quickly cause sepsis and kidney damage. Antibiotics alone will not clear it. The kidney needs urgent decompression, usually with a nephrostomy or ureteric stent.

Risposta

B · Urgent drainage of the kidney plus IV antibiotics
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: C · Arterial insufficiency

Perché è questa la risposta

Rest pain eased by dependency is typical of severe peripheral arterial disease. Elevating the leg worsens pain, and compression is avoided. Vascular assessment is needed.

Risposta

C · Arterial insufficiency
