1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

Risposta

C · Giallo (differito)
2.Priorità e delega

Risposta: D · Stop the infusion and inform the provider at once

Perché è questa la risposta

A new severe headache, vomiting or neurological decline during thrombolysis can mean intracranial bleeding. The infusion is stopped, the provider informed and an urgent scan arranged.

Risposta

D · Stop the infusion and inform the provider at once
3.Controllo delle infezioni e sicurezza del paziente

Risposta: C · The patient’s allergies, by checking the record and asking the patient

Perché è questa la risposta

Before any new drug, the nurse checks recorded allergies and asks the patient directly about past reactions. The patient is then watched closely after the first dose.

Risposta

C · The patient’s allergies, by checking the record and asking the patient
4.Controllo delle infezioni e sicurezza del paziente

Risposta: C · Lead poisoning from old paint dust or chips

Perché è questa la risposta

Lead-based paint in older housing is the main source of lead exposure in young children, who put dust and chips in their mouths. Lead can cause anemia, behavior changes and developmental problems, and a blood lead level confirms it.

Risposta

C · Lead poisoning from old paint dust or chips
5.Valori di laboratorio ed elettroliti

Risposta: C · Liver function tests and platelet count

Perché è questa la risposta

Valproate can cause liver toxicity and thrombocytopenia, so liver tests and the platelet count are monitored. It can also raise ammonia; confusion or vomiting should prompt an ammonia level.Verificato con: Valproic Acid · StatPearls (NCBI Bookshelf) (2024)

Risposta

C · Liver function tests and platelet count
6.Valori di laboratorio ed elettroliti

Risposta: D · Multiple myeloma

Perché è questa la risposta

Multiple myeloma typically causes high calcium from bone destruction, kidney damage, anemia and bone lesions, plus a raised protein from a monoclonal antibody. Recurrent infections reflect poor production of normal antibodies.Verificato con: Multiple Myeloma · StatPearls (NCBI Bookshelf) (2023)

Risposta

D · Multiple myeloma
7.Infermieristica clinica

Risposta

A · Trattarlo come sepsi neutropenica e somministrare con urgenza antibiotici EV
8.Infermieristica clinica

Risposta: C · A catheter-related bloodstream infection

Perché è questa la risposta

Rigors after flushing suggest bacteria colonising the catheter. Blood cultures are taken (often from the line and a peripheral vein) and the team decides whether to remove the line.

Risposta

C · A catheter-related bloodstream infection
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: B · Remove the cannula, document and monitor the site

Perché è questa la risposta

Redness, pain, warmth and swelling along the vein indicate phlebitis. The cannula is removed, the site is observed for infection and the event is documented. Regular visual phlebitis scoring and removing cannulas that are no longer needed reduce the risk.

Risposta

B · Remove the cannula, document and monitor the site
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: D · A low-dose opioid with a hand-held fan and calm positioning

Perché è questa la risposta

Low-dose oral opioids can ease refractory breathlessness in advanced disease. A fan blowing air across the face, upright positioning, breathing techniques and anxiety management also help. Oxygen is used only if it relieves symptoms or corrects hypoxaemia.

Risposta

D · A low-dose opioid with a hand-held fan and calm positioning
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: A · A beta-blocker alone can cause a dangerous rise in BP

Perché è questa la risposta

Blocking beta receptors alone leaves catecholamines acting on alpha receptors, causing severe vasoconstriction and a hypertensive crisis. Alpha blockade, such as with phenoxybenzamine, comes first, and a beta-blocker may be added later for tachycardia.

Risposta

A · A beta-blocker alone can cause a dangerous rise in BP
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: D · Overdrainage of CSF, so act per protocol and inform the team

Perché è questa la risposta

When the head rises above the drain level, CSF drains faster. Overdrainage can cause headache, collapse of the ventricles or a subdural bleed. The nurse follows the protocol (often clamping briefly and re-levelling) and informs the neurosurgical team.

Risposta

D · Overdrainage of CSF, so act per protocol and inform the team
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · They reduce the feeling of breathlessness

Perché è questa la risposta

In palliative care, low-dose opioids reduce the sensation of breathlessness and the anxiety it causes. Fans, positioning, relaxation and treating reversible causes are also used.

Risposta

D · They reduce the feeling of breathlessness
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · Cancer and its treatment greatly increase clot risk

Perché è questa la risposta

Cancer, chemotherapy, central lines and immobility increase the risk of DVT and PE. Unilateral leg swelling needs assessment with ultrasound and possible anticoagulation.

Risposta

D · Cancer and its treatment greatly increase clot risk
