1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

  2. BMassage the area firmly to spread the drug out into the surrounding tissue
  3. CStop it at that site, inform the team and follow the extravasation policy
  4. DIncrease the rate so the drug enters the vein faster and leaks less
11.Area medico-chirurgica: neurologica, endocrina e renaleMedia

A patient receives phenytoin through a nasogastric tube along with continuous enteral feed. Which issue should the nurse know about?

  1. AThe feed can reduce phenytoin absorption, so the feed is often paused around the dose
  2. BPhenytoin must be crushed and mixed into the feed bag
  3. CThe feed increases phenytoin levels and causes toxicity within hours

3Soluzioni

1.Priorità e delega

Risposta: B · Matching each patient’s needs to staff competence

Perché è questa la risposta

Assignments are based mainly on patient acuity and needs and on each staff member’s skills and scope. Geography and continuity of care help, but they come after safety.

Risposta

B · Matching each patient’s needs to staff competence

Risposta: D · The surface stays wet for the contact time on the product label

Perché è questa la risposta

Each disinfectant has a contact (dwell) time during which the surface must stay visibly wet. Drying it early or spreading one wipe over many items reduces its effect.

Risposta

D · The surface stays wet for the contact time on the product label
4.Controllo delle infezioni e sicurezza del paziente

Risposta: D · “We will put the rear-facing seat in the front so we can see the baby.”

Perché è questa la risposta

A rear-facing car seat must never be placed in front of an active airbag, which can strike the seat with great force. Infants ride rear-facing in the back seat, with a snug harness, on every trip.

Risposta

D · “We will put the rear-facing seat in the front so we can see the baby.”
5.Valori di laboratorio ed elettroliti

Risposta: A · Plasma volume rises more than red cell mass, diluting the blood

Perché è questa la risposta

In pregnancy, plasma volume increases by more than red cell mass, so the hemoglobin falls slightly (physiologic or dilutional anemia). Iron deficiency is still common in pregnancy and is checked with the MCV and ferritin.Verificato con: Anemia · StatPearls (NCBI Bookshelf) (2026)

Risposta

A · Plasma volume rises more than red cell mass, diluting the blood
6.Valori di laboratorio ed elettroliti

Risposta: C · Spontaneous bacterial peritonitis

Perché è questa la risposta

An ascitic neutrophil count of 250 cells/mm³ or more indicates spontaneous bacterial peritonitis, which is treated with antibiotics. It can occur without classic signs, so new confusion or worsening kidney function in cirrhosis should also prompt a diagnostic paracentesis.Verificato con: Ascites · MedlinePlus (NIH) (2024); Peritonitis · MedlinePlus (NIH) (2026)

Risposta

C · Spontaneous bacterial peritonitis
7.Infermieristica clinica

Risposta: A · Category 4

Perché è questa la risposta

Category 4 involves full-thickness tissue loss with exposed bone, tendon or muscle. Category 3 is full-thickness skin loss without exposed bone or tendon. US (NPIAP) terminology calls these stages rather than categories.

Risposta

A · Category 4
8.Infermieristica clinica

Risposta: D · A haematoma compressing the airway

Perché è questa la risposta

A rapidly expanding neck haematoma after thyroid surgery is an airway emergency. Call for urgent help; local protocols often require equipment to release the wound clips or sutures to be kept at the bedside.

Risposta

D · A haematoma compressing the airway
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: B · Regular sawtooth flutter waves between QRS complexes

Perché è questa la risposta

Atrial flutter is caused by a re-entry circuit in the right atrium, giving regular sawtooth flutter waves, often at about 300/min with 2:1 block (ventricular rate about 150/min). A chaotic baseline with irregular QRS is atrial fibrillation. Like AF, it carries a stroke risk.

Risposta

B · Regular sawtooth flutter waves between QRS complexes
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: C · Stop it at that site, inform the team and follow the extravasation policy

Perché è questa la risposta

Extravasated noradrenaline causes intense local vasoconstriction and tissue necrosis. The infusion is stopped at that site (with a plan to continue elsewhere, since the patient depends on it), the team is informed and the local extravasation protocol is followed. Vasopressors are given through a central line where possible.

Risposta

C · Stop it at that site, inform the team and follow the extravasation policy
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: A · The feed can reduce phenytoin absorption, so the feed is often paused around the dose

Perché è questa la risposta

Enteral feeds can bind phenytoin and greatly reduce how much is absorbed. Local protocols usually pause the feed before and after the dose and flush the tube. Drug levels are monitored.

Risposta

A · The feed can reduce phenytoin absorption, so the feed is often paused around the dose
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: C · Hypoactive delirium

Perché è questa la risposta

Hypoactive delirium presents with drowsiness, withdrawal and slowness rather than agitation, so it is often missed. It has a worse prognosis than the hyperactive form. A family report of a sudden change is important, and a screening tool such as the 4AT or the CAM (Confusion Assessment Method) can be used.

Risposta

C · Hypoactive delirium
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · Circa 5–10 giorni dopo l'intervento

Risposta

D · Circa 5–10 giorni dopo l'intervento
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: A · Possibile compressione midollare: allertare, steroidi come prescritto, RM urgente

Risposta

A · Possibile compressione midollare: allertare, steroidi come prescritto, RM urgente
