1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: C · Two patients who both have C. difficile infection

Perché è questa la risposta

Cohorting patients with the same organism is acceptable when single rooms are not available. The other pairs would expose a vulnerable patient to an infection.

Risposta

C · Two patients who both have C. difficile infection
2.Priorità e delega

Risposta: C · Give five rescue breaths; if breathing starts, tag the child red

Perché è questa la risposta

In JumpSTART, an apneic child with a pulse gets five rescue breaths, because the cause is often respiratory. If breathing starts the child is immediate (red); if not, the child is expectant.

Risposta

C · Give five rescue breaths; if breathing starts, tag the child red
3.Controllo delle infezioni e sicurezza del paziente

Risposta: D · The prep must be fully dry and any pooled liquid removed

Perché è questa la risposta

Wet alcohol prep and its vapors under drapes can be ignited by electrosurgery or lasers. The prep is allowed to dry fully, and pooling in skin folds or linen is prevented.

Risposta

D · The prep must be fully dry and any pooled liquid removed
4.Controllo delle infezioni e sicurezza del paziente

Risposta: D · “I keep small rugs in the hall so my feet stay warm.”

Perché è questa la risposta

Loose rugs are a common cause of trips and falls and should be removed or secured. Night lights, grab bars and non-slip footwear all reduce fall risk.

Risposta

D · “I keep small rugs in the hall so my feet stay warm.”
5.Valori di laboratorio ed elettroliti

Risposta: B · Have the patient void and discard that urine, then save all urine until 08:00 the next day

Perché è questa la risposta

The collection starts with an empty bladder, so the first void is discarded and the time recorded. All urine for the next 24 hours is saved, including a final void at the end time. If urine is discarded by mistake, the collection usually has to restart.Verificato con: Urine 24-hour volume · MedlinePlus (NIH) (2025)

Risposta

B · Have the patient void and discard that urine, then save all urine until 08:00 the next day
6.Valori di laboratorio ed elettroliti

Risposta: A · Subclinical hypothyroidism

Perché è questa la risposta

A raised TSH with a normal free T4 is subclinical hypothyroidism; the pituitary is working harder to keep T4 normal. Overt hypothyroidism has a low free T4, and subclinical hyperthyroidism has a low TSH.Verificato con: TSH (Thyroid-stimulating hormone) Test · MedlinePlus (NIH) (2024)

Risposta

A · Subclinical hypothyroidism
7.Infermieristica clinica

Risposta: D · Dull percussion and reduced breath sounds over the lung base

Perché è questa la risposta

Fluid in the pleural space muffles breath sounds and makes percussion stony dull. A large effusion may push the trachea away from the affected side.

Risposta

D · Dull percussion and reduced breath sounds over the lung base
8.Infermieristica clinica

Risposta: A · Paralytic ileus

Perché è questa la risposta

Paralytic ileus is a temporary loss of gut movement, common after abdominal surgery. Bowel sounds are absent, unlike mechanical obstruction where they are often high-pitched. Management includes nil by mouth, NG decompression, fluids and correcting electrolytes.

Risposta

A · Paralytic ileus
9.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: A · Some warning signs, such as a fast heartbeat, may be masked

Perché è questa la risposta

Beta-blockers can blunt adrenergic symptoms of hypoglycaemia such as palpitations and tremor. Sweating usually remains. Patients should check their glucose if they feel unwell and know their other warning signs.

Risposta

A · Some warning signs, such as a fast heartbeat, may be masked
10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta

C · Allertare subito come emergenza; tenere la gamba orizzontale, senza calore
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: A · No antibiotics, as long as there are no symptoms

Perché è questa la risposta

Nearly all long-term catheters become colonised. Treating asymptomatic bacteriuria does not improve outcomes and promotes resistance. Antibiotics are reserved for symptomatic infection.

Risposta

A · No antibiotics, as long as there are no symptoms
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta

C · Ritenzione urinaria; cateterizzare come prescritto e osservare una grande diuresi
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: B · To prevent Wernicke's encephalopathy

Perché è questa la risposta

Thiamine deficiency is common in alcohol dependence and can cause Wernicke's encephalopathy (confusion, ataxia, eye signs). Thiamine is given early, ideally before or with glucose, as glucose can worsen deficiency.

Risposta

B · To prevent Wernicke's encephalopathy
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · Refer to the dietitian, start a nutrition plan and monitor intake

Perché è questa la risposta

A MUST score of 2 or more indicates high risk of malnutrition. Poor nutrition delays healing, so the nurse refers to the dietitian, starts a care plan with food charts and supplements as advised, and rescreens regularly.

Risposta

D · Refer to the dietitian, start a nutrition plan and monitor intake
