1Formule e metodi

Priorità e delega

Metodo

Valutazione, educazione e verifica restano all’infermiere.

Controllo delle infezioni e sicurezza del paziente

Metodo

  2. Aggiungi precauzioni in base alla via: contatto (camice, guanti), goccioline (mascherina chirurgica), via aerea (N95, stanza a pressione negativa).
  4. Previeni i danni nell’ambiente: misure anticaduta, contenzione solo come ultima risorsa, RACE e PASS per l’incendio.

Area medico-chirurgica: cardiovascolare e respiratoria

Metodo

  5. Scegli la prima azione sicura, poi intensifica l’intervento e rivaluta.

Area medico-chirurgica: neurologica, endocrina e renale

Metodo

Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Metodo

3Soluzioni

1.Priorità e delega

Risposta: A · Delirium, which needs the cause found quickly

Perché è questa la risposta

Delirium starts suddenly, over hours or days, and attention fluctuates. It often has a treatable cause such as infection, hypoxia or medication, so it is a priority to assess.

Risposta

A · Delirium, which needs the cause found quickly
2.Priorità e delega

Risposta: B · Not witness the consent and inform the surgeon

Perché è questa la risposta

Valid consent requires a patient who can understand and decide. After a sedative this may not be possible, so the nurse does not witness it and informs the surgeon, who is responsible for obtaining consent.

Risposta

B · Not witness the consent and inform the surgeon
3.Controllo delle infezioni e sicurezza del paziente

Risposta: B · On the hands, with the crutch tops a few finger widths below the armpits

Perché è questa la risposta

Weight is taken on the hands and arms. Leaning on the crutch tops can compress the brachial plexus and cause crutch palsy, so a gap is kept between the crutch top and the axilla.

Risposta

B · On the hands, with the crutch tops a few finger widths below the armpits
4.Controllo delle infezioni e sicurezza del paziente

Risposta: B · Keep one up-to-date list of all medicines and use one pharmacy where possible

Perché è questa la risposta

A single complete medication list and one pharmacy allow interactions and duplicates to be detected. The list, including over-the-counter drugs and supplements, is shown at every visit.

Risposta

B · Keep one up-to-date list of all medicines and use one pharmacy where possible
5.Valori di laboratorio ed elettroliti

Risposta: B · A common stress response to surgery that is monitored

Perché è questa la risposta

A mild leukocytosis is common after surgery because of tissue injury and the stress response. It is monitored along with temperature and wound assessment. A rising count after several days, or fever, suggests infection.Verificato con: WBC count · MedlinePlus (NIH) (2025)

Risposta

B · A common stress response to surgery that is monitored
6.Valori di laboratorio ed elettroliti

Risposta: C · A sample taken from the wrong patient

Perché è questa la risposta

A sudden large change with no clinical change suggests a specimen error, such as wrong blood in the tube or dilution with IV fluid. The nurse assesses the patient, informs the provider and arranges a repeat sample with careful identification, while staying alert for true bleeding.

Risposta

C · A sample taken from the wrong patient
7.Infermieristica clinica

Risposta: D · Bending the hip beyond 90 degrees or crossing the legs

Perché è questa la risposta

Hip flexion past 90°, crossing the legs and turning the leg inwards can dislocate the hip after a posterior approach. Raised seats and toilet seats help, although specific precautions vary between units.

Risposta

D · Bending the hip beyond 90 degrees or crossing the legs
8.Infermieristica clinica

Risposta: B · A patient who has just become breathless after starting an IV antibiotic

Perché è questa la risposta

Breathlessness soon after a new IV antibiotic suggests possible anaphylaxis, an airway and breathing emergency. The infusion should be stopped and the patient assessed immediately.

Risposta

B · A patient who has just become breathless after starting an IV antibiotic
9.Area medico-chirurgica: cardiovascolare e respiratoria

10.Area medico-chirurgica: cardiovascolare e respiratoria

Risposta: A · Check vital signs, report urgently and anticipate a blood count

Perché è questa la risposta

Melaena with postural dizziness suggests upper gastrointestinal bleeding, a risk with dual antiplatelet therapy. The nurse assesses for shock, informs the team urgently and expects bloods, a group and save (type and screen) and possibly endoscopy. Stopping antiplatelets after a stent risks stent thrombosis, so the decision belongs to the medical and cardiology team.

Risposta

A · Check vital signs, report urgently and anticipate a blood count
11.Area medico-chirurgica: neurologica, endocrina e renale

Risposta: B · Glucose tablets or a small glass of fruit juice

Perché è questa la risposta

About 15–20 g of fast-acting carbohydrate, such as glucose tablets or juice, raises glucose quickly. Chocolate and milk contain fat, which slows absorption. Glucose is rechecked after about 15 minutes.

Risposta

B · Glucose tablets or a small glass of fruit juice
12.Area medico-chirurgica: neurologica, endocrina e renale

Risposta

B · Trattare la glicemia bassa e rivalutare i segni neurologici
13.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: C · Withdrawal occurs if the drug is stopped suddenly

Perché è questa la risposta

Physical dependence is a predictable adaptation to regular opioids, causing withdrawal if they stop suddenly, so doses are tapered. Addiction is a behavioural condition with craving, loss of control and continued use despite harm.

Risposta

C · Withdrawal occurs if the drug is stopped suddenly
14.Area medico-chirurgica: gastrointestinale, perioperatoria e oncologica

Risposta: D · It raises the risk of ketoacidosis, even with near-normal glucose

Perché è questa la risposta

SGLT2 inhibitors can cause euglycaemic diabetic ketoacidosis around fasting and surgery. They are usually paused before surgery as advised, and ketones are checked if the patient becomes unwell.

Risposta

D · It raises the risk of ketoacidosis, even with near-normal glucose
