PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 を使って A–a 勾配 を求めてください。

After surgery, a patient who has taken long-term steroids becomes hypotensive and weak, and the blood pressure does not rise with IV fluids. Which problem should the nurse suspect?
B · A swallowing assessment and sitting upright for meals
Patients with dysphagia need a formal swallowing assessment, food and fluid textures that are safe for them, sitting upright during and after meals and close supervision. Good oral hygiene also reduces the bacterial load that can be aspirated.
A · Early haemorrhagic shock is likely, so escalate urgently
Rising heart rate, restlessness, pallor and visible bleeding are signs of compensated haemorrhagic shock. Waiting for hypotension delays treatment. The nurse escalates urgently, gives oxygen, ensures IV access and anticipates bloods, fluids or blood products and surgical review.
B · A beta-blocker alone can cause a dangerous rise in BP
Blocking beta receptors alone leaves catecholamines acting on alpha receptors, causing severe vasoconstriction and a hypertensive crisis. Alpha blockade, such as with phenoxybenzamine, comes first, and a beta-blocker may be added later for tachycardia.
D · Stop the test and inform the doctor
In DI, the patient keeps passing dilute urine despite fluid restriction and can become dangerously dehydrated. Significant weight loss, hypotension or a high sodium are reasons to stop the test and get medical review.
C · "I can cut the patch in half if I need less."
Cutting a fentanyl patch can release the drug unpredictably and cause overdose. Heat increases absorption, used patches still contain drug and must be folded and disposed of safely, and sites are rotated.
B · Postural hypotension
A fall in systolic pressure of 20 mmHg or more (here 26 mmHg) on standing, especially with symptoms, indicates postural hypotension. Causes include medicines, dehydration and autonomic problems.
D · Biotin can distort some immunoassays, such as troponin or TSH
Many immunoassays use biotin-streptavidin binding, and high blood biotin levels can give falsely high or low results, for example a falsely low troponin or abnormal thyroid tests. The laboratory and prescriber should know about biotin use.
C · "I'll take my iron tablet with a big glass of milk"
Calcium in milk, as well as antacids and tea, reduces iron absorption, so iron should not be taken with them. Vitamin C improves absorption, dark stools are expected, and iron overdose is a serious poisoning risk in young children.
PaCO₂ = 26 mmHg
答え: A–a 勾配 = 134.7 mmHg
海面での肺胞気酸素分圧は PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 mmHg で、A–a 勾配は PAO₂ − PaO₂ です。
A · Adrenal crisis from steroid withdrawal
Patients on long-term steroids may not make enough cortisol to cope with surgical stress. Refractory hypotension suggests adrenal crisis, which needs urgent IV hydrocortisone as prescribed.
