A patient with long-standing rheumatoid arthritis has a hemoglobin of 10.5 g/dL (105 g/L), a low serum iron, a low total iron-binding capacity (TIBC) and a high-normal ferritin. Which condition fits best?
PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 を使って A–a 勾配 を求めてください。
A malnourished patient admitted with alcohol use disorder needs IV glucose. Which vitamin is given before or with it?
After the RN taught a patient to inject insulin, an LPN/LVN reports that the patient still cannot draw up the dose correctly. What should happen next?
C · Sit down for tasks such as washing and dressing, and pace activities
Planning and pacing, sitting for tasks, keeping frequently used items within reach and breathing out during effort reduce breathlessness. Complete inactivity worsens deconditioning, so activity is encouraged within limits.
D · A low-dose opioid with a hand-held fan and calm positioning
Low-dose oral opioids can ease refractory breathlessness in advanced disease. A fan blowing air across the face, upright positioning, breathing techniques and anxiety management also help. Oxygen is used only if it relieves symptoms or corrects hypoxaemia.
B · TURP syndrome from absorbed irrigation fluid
In TURP syndrome, irrigating fluid is absorbed into the blood, causing fluid overload and dilutional hyponatraemia. Signs include confusion, nausea, bradycardia, hypertension and, if severe, seizures. It needs urgent medical treatment.
B · Patients can be seriously injured climbing over or becoming trapped in rails
Bed rails can cause entrapment or a fall from a greater height. Their use is based on an individual risk assessment; alternatives include a low bed, crash mats and closer observation.
A · Escalate urgently, as the patient may be bleeding
A large fall in haemoglobin with tachycardia suggests ongoing blood loss. The nurse assesses the patient with ABCDE, checks wounds and drains and escalates for urgent review.
D · Anemia of chronic disease (inflammation)
In chronic inflammation, iron is trapped in stores, so serum iron is low but ferritin is normal or high and TIBC is low. In iron deficiency, ferritin is low and TIBC is high. Iron supplements help iron deficiency much more than anemia of chronic disease. Ferritin Blood Test · MedlinePlus (NIH) (2025); Serum iron test · MedlinePlus (NIH) (2026)
Osm = 350 mOsm/kg
海面での肺胞気酸素分圧は PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 mmHg で、A–a 勾配は PAO₂ − PaO₂ です。
答え: A–a 勾配 = 104.2 mmHg
海面での肺胞気酸素分圧は PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 mmHg で、A–a 勾配は PAO₂ − PaO₂ です。
B · Thiamine (vitamin B1)
Thiamine deficiency is common in heavy alcohol use, and giving glucose without thiamine can trigger Wernicke encephalopathy. Thiamine is given before or with glucose.
B · The RN reassesses the patient’s learning needs and adjusts the teaching
Evaluating learning and changing the teaching plan belong to the RN. The LPN/LVN can reinforce teaching, but a teaching plan that is not working needs RN reassessment.
