Four stable patients need routine morning care. To reduce the spread of infection, which patient should the nurse visit first?
B · Pneumonia or cancer affecting the pleura
Transudates form when hydrostatic or oncotic pressure changes push fluid out of vessels, as in heart failure, cirrhosis and nephrotic syndrome. Exudates form when inflammation or tumour makes the pleura leaky, so protein and LDH are high (Light's criteria). Common causes are pneumonia, cancer, TB and pulmonary embolism.
D · Atrial fibrillation
New atrial fibrillation affects a large proportion of patients in the first days after cardiac surgery, often due to inflammation, electrolyte shifts and stress. Management includes a 12-lead ECG, checking potassium and magnesium, rate or rhythm control and considering anticoagulation.
C · It contains concentrated anticoagulant
Dialysis catheter lumens are filled with a concentrated heparin or citrate lock. If it is flushed into the patient, it can cause bleeding or other effects. Catheters are usually accessed only by trained staff.
A · Constipation
Constipation, GI bleeding, infection, dehydration, low potassium and sedative drugs can trigger encephalopathy. Keeping the bowels open, for example with lactulose, reduces ammonia absorption.
D · "Peppermint tea after dinner will settle my reflux."
Peppermint can relax the lower oesophageal sphincter and may worsen reflux. Weight loss, stopping smoking and avoiding personal triggers are standard lifestyle measures.
D · Attacks of symptoms, such as visual loss or numbness, that improve between relapses
MS causes demyelination in the brain and spinal cord. Many patients first have attacks such as optic neuritis or sensory symptoms, with partial or full recovery between relapses.
B · Paralytic ileus
Paralytic ileus is a temporary loss of gut movement, common after abdominal surgery. Bowel sounds are absent, unlike mechanical obstruction where they are often high-pitched. Management includes nil by mouth, NG decompression, fluids and correcting electrolytes.
A · Average glucose over the past months has been above the target
An HbA1c of 7.8% shows that average glucose has been above the individual target over about 2–3 months. The nurse explores diet, activity, medication use and barriers, and the treatment plan may be adjusted.
A · Add dextrose to the IV fluids and continue the insulin
The glucose usually falls before the ketoacidosis resolves. Dextrose is added so insulin can continue, clearing ketones without causing hypoglycemia, until the anion gap closes. Stopping insulin too early lets ketoacidosis return.
答え: A–a 勾配 = 199.2 mmHg
海面での肺胞気酸素分圧は PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 mmHg で、A–a 勾配は PAO₂ − PaO₂ です。
D · A patient with neutropenia after chemotherapy
When urgency is equal, the nurse cares for the most vulnerable patient first and patients with transmissible infections last. This lowers the risk of carrying organisms to an immunocompromised patient.
