A patient was knocked out briefly in a fall, then woke and talked normally. Two hours later the patient becomes drowsy and one pupil is larger. What is the most likely cause?
D · Raised above the level of the heart when resting
Elevating the legs above heart level reduces venous pressure and oedema. Compression therapy, walking and avoiding long periods of standing also help. By contrast, patients with arterial disease often find that lowering the legs eases pain.
A · Stop suctioning, give oxygen and monitor the patient
Suctioning can cause hypoxaemia and vagal stimulation, leading to bradycardia and arrhythmias. The nurse stops, gives oxygen and observes. Each pass should be short (usually no more than about 10–15 seconds), with pre-oxygenation if needed.
A · Late doses can soon cause stiffness, falls and poor swallowing
People with Parkinson's disease often take doses at exact times. A delay of even 30 minutes or so can cause rigidity, immobility, falls and unsafe swallowing. Many hospitals treat these drugs as time-critical.
A · IV hydrocortisone straight away, plus IV fluids
An adrenal crisis is life-threatening. IV or IM hydrocortisone is given immediately, without waiting for tests, together with IV saline and checks of glucose and electrolytes. Vomiting prevents tablets from being absorbed.
B · Full-thickness loss with exposed bone, tendon or muscle
Category 4 has full-thickness loss with exposed bone, tendon or muscle and often undermining or tunnelling. Category 3 reaches the fat but does not expose bone, tendon or muscle.
D · Cardiac tamponade from a pericardial effusion
Malignant pericardial effusion can compress the heart, causing hypotension, raised JVP and muffled heart sounds. It is an emergency needing urgent echocardiography and drainage (pericardiocentesis).
B · The risk of a full stroke is highest in the days after a TIA
The risk of stroke is highest soon after a TIA. Rapid assessment and treatment, such as antiplatelets, statins and carotid imaging, reduce this risk.
Heart failure - fluids and diuretics · MedlinePlus (NIH); Low blood potassium · MedlinePlus (NIH) (2025)
A · Cortisol is being produced without normal feedback control
Dexamethasone normally suppresses ACTH and lowers morning cortisol. Failure to suppress suggests autonomous cortisol excess (Cushing syndrome). Further tests then find the source, such as a pituitary or adrenal tumor.
Osm = 323 mOsm/kg
海面での肺胞気酸素分圧は PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 mmHg で、A–a 勾配は PAO₂ − PaO₂ です。
A · Raise the head of the bed, give oxygen and call for urgent help
Sudden breathlessness and chest pain after surgery suggest a pulmonary embolism. The nurse supports breathing with positioning and oxygen and escalates at once; walking or leg massage could make things worse.
A · An epidural hematoma
An epidural hematoma often causes a brief loss of consciousness, a lucid interval, then rapid deterioration as arterial bleeding compresses the brain. It is a neurosurgical emergency.
