On the first day after abdominal surgery, a patient has a temperature of 37.9 °C and reduced breath sounds at the lung bases. Which nursing action is most helpful?
Which nursing approach may raise intracranial pressure in a patient with a head injury?
D · Statins reduce further events whatever the starting cholesterol
After ACS, high-intensity statin therapy reduces recurrent MI, stroke and death regardless of baseline cholesterol, partly by stabilising plaques. It is part of standard secondary prevention with antiplatelets, beta-blockers, ACE inhibitors and lifestyle change.
A · Stop smoking and do not fly until a doctor confirms full resolution
Patients should not fly until the pneumothorax has resolved on X-ray, often with an extra waiting period. Diving is usually avoided permanently unless definitive surgery has been done. Smoking increases recurrence, which is common, so patients learn to seek help for new breathlessness or chest pain.
B · To improve the strength of chewing and swallowing at mealtimes
Pyridostigmine inhibits acetylcholinesterase, so more acetylcholine is available at the neuromuscular junction. Timing it before meals gives the best muscle strength for chewing and swallowing, which lowers the risk of aspiration.
D · An NSAID such as diclofenac
NSAIDs are recommended first line for renal colic in suitable patients because they reduce prostaglandin-driven pain and ureteric spasm. Opioids are added if needed. NSAIDs are avoided in kidney impairment.
B · An intramuscular injection
IM injections can cause haematomas in severe thrombocytopenia. Rectal procedures, NSAIDs and aspirin are also avoided. The nurse checks with the prescriber and the oral or IV route is used where possible.
C · To exclude significant arterial disease, as compression could cut off blood flow
ABPI compares ankle and arm blood pressures. A low result suggests arterial insufficiency, where compression could cause ischaemia; venous ulcers with adequate arterial supply are treated with compression.
D · "My output will be firm and formed like before."
Ileostomy output is liquid to porridge-like because it bypasses the colon. The other statements describe correct care: good hydration, emptying the bag before it is too heavy and recognising a healthy stoma.
B · Pseudohyponatremia, an artifact caused by very high lipids
Very high lipids or proteins take up part of the plasma volume, so some laboratory methods report a falsely low sodium. The osmolality and the sodium measured by direct methods are normal. Treating this "hyponatremia" would be harmful.
PaCO₂ = 46.9 mmHg
D · Encourage deep breathing, coughing and use of the incentive spirometer
Reduced basal breath sounds and a mild temperature early after surgery often mean atelectasis. Deep breathing, coughing, incentive spirometry and early mobilization help re-expand the lungs.
B · Doing suctioning, turning and bathing all at the same time
Clustering several stimulating activities can cause a sustained rise in intracranial pressure. Spacing care, keeping the neck straight and a calm environment help keep it down.
