An older adult with type 2 diabetes is confused and very dehydrated. The glucose is 900 mg/dL (50 mmol/L), and blood and urine show no significant ketones. What is the most likely problem?
A · Stop suctioning, give oxygen and monitor the patient
Suctioning can cause hypoxaemia and vagal stimulation, leading to bradycardia and arrhythmias. The nurse stops, gives oxygen and observes. Each pass should be short (usually no more than about 10–15 seconds), with pre-oxygenation if needed.
D · Strict fluid balance, daily weight and free access to water
Patients with DI can become dehydrated and hypernatraemic quickly. Strict input and output, daily weights, sodium monitoring and access to water are essential. Desmopressin replaces the missing ADH in cranial DI.
C · Pain, nausea, agitation and respiratory secretions
Anticipatory medicines are prescribed in advance, usually for subcutaneous use, so that common end-of-life symptoms can be treated quickly without delay, especially at home.
C · Inform the anaesthetist, as malignant hyperthermia may run in the family
Malignant hyperthermia susceptibility is inherited. The anaesthetist plans a trigger-free anaesthetic, avoiding volatile agents and suxamethonium, and the family may be referred for testing.
D · Metabolic acidosis
In DKA, ketoacids consume bicarbonate, causing a metabolic acidosis. The patient breathes deeply and fast (Kussmaul breathing) to lower PaCO2 as compensation.
A · Give another dose of fast-acting carbohydrate and recheck
If blood glucose remains below the threshold (4 mmol/L in UK guidance, 70 mg/dL in US guidance), the fast-acting treatment is repeated. Once it is above the threshold, a longer-acting carbohydrate such as a sandwich or the next meal helps prevent recurrence.
Partial Thromboplastin Time (PTT) Test · MedlinePlus (NIH) (2026)
Osmotic demyelination syndrome · MedlinePlus (NIH) (2026)
AGc = 18.5 mmol/L
A · Get the patient some carbohydrate food now and check the glucose
Rapid-acting insulin starts working within minutes, so a long delay before eating risks hypoglycemia. The nurse arranges food or carbohydrate now and monitors the glucose.
D · Hyperosmolar hyperglycemic state
Hyperosmolar hyperglycemic state occurs mainly in type 2 diabetes, with extreme glucose, severe dehydration and little or no ketosis. Treatment starts with IV fluids, as in DKA.
