A nursing assistant offers to suction a patient’s new tracheostomy. Which response from the RN is best?
A patient with septic shock remains hypotensive, with a mean arterial pressure of 58 mmHg, after adequate IV fluids. What should the nurse anticipate?
B · A nitrate-free interval prevents tolerance
Continuous nitrate use leads to tolerance within days. A daily nitrate-free period (often overnight, when angina is least likely) keeps the drug effective. Patients are taught to rotate patch sites.
A · Low oxygen saturation is not explained by anxiety; PE must be excluded
Panic-related hyperventilation causes respiratory alkalosis but normal oxygen saturation. Hypoxaemia with tachycardia in a woman taking oestrogen suggests PE. She needs oxygen, urgent medical assessment and investigation. Paper-bag rebreathing is unsafe and can worsen hypoxaemia.
B · IV fluids to rehydrate the patient
Hypercalcaemia causes polyuria and dehydration, which worsen the high calcium. IV fluids are the first step, followed by a bisphosphonate to reduce bone resorption. Calcium is rechecked regularly.
B · Jaundice, dark urine, pale stools and itching
When bile cannot reach the gut, conjugated bilirubin enters the blood and urine (dark urine), stools lose their colour, and bile salts in the skin cause itching. Causes include gallstones and pancreatic cancer.
B · Residual neuromuscular blockade
Remaining muscle relaxant causes weakness, poor head lift and shallow breathing in an awake patient. The nurse supports breathing, gives oxygen and calls the anaesthetist, who may give a reversal agent.
D · Increased urine output, less breathlessness and a falling weight
Effective diuresis increases urine output and reduces congestion, so breathing improves and weight falls. Electrolytes, renal function and blood pressure are also monitored.
D · The combination can cause a severe, dangerous fall in blood pressure
PDE5 inhibitors such as sildenafil enhance the vasodilating effect of nitrates, which can cause profound hypotension. Patients should always be asked about these drugs before nitrates are given.
A · A low serum sodium
Carbamazepine (and oxcarbazepine) can cause hyponatremia by increasing the effect of ADH, which can cause confusion, headache and seizures. The drug can also cause serious blood disorders, so the CBC is monitored.
D · "I can have a light breakfast before the test"
The OGTT is done after an overnight fast of at least 8 hours; water is allowed. Eating before the test makes the results invalid. Patients rest during the test and avoid smoking, which can affect glucose.
Osm = 340 mOsm/kg
海面での肺胞気酸素分圧は PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 mmHg で、A–a 勾配は PAO₂ − PaO₂ です。
AGc = 24.75 mmol/L
答え: CPR(電気ショックの適応とならないリズム)
C · “Suctioning a new tracheostomy is my task; I will do it myself.”
Care of a new tracheostomy, including suctioning, involves airway risk and assessment, so the RN keeps it. Leaving it to the family or the next shift is unsafe.
C · A vasopressor, such as norepinephrine, to raise the pressure
If hypotension continues after fluid resuscitation, a vasopressor such as norepinephrine is started to keep the MAP at least 65 mmHg. Diuretics and beta-blockers would worsen perfusion.
