A day after a femur fracture, a young patient becomes confused and breathless, with small red spots on the chest. What should the nurse suspect?
A · HDL carries cholesterol from the tissues back to the liver
HDL transports cholesterol from the tissues and artery walls to the liver for removal (reverse cholesterol transport), so higher levels are linked with lower cardiovascular risk. LDL carries cholesterol to tissues and drives atherosclerosis.
A · A parapneumonic effusion or empyema
Persistent fever and signs of pleural fluid suggest a parapneumonic effusion, which may become infected (empyema). Ultrasound and diagnostic pleural aspiration are needed, and an infected collection usually needs a chest drain in addition to antibiotics.
C · Sugar on the skin can give a falsely high reading
Traces of sugar on the fingers can falsely raise the reading. Hands are washed and dried before testing. The side of the fingertip is used, and lancets are single use.
C · The dose is too high
A suppressed TSH with raised free T4 and hyperthyroid symptoms in a patient on levothyroxine suggests over-replacement. The prescriber usually lowers the dose. Long-term over-replacement increases the risk of atrial fibrillation and bone loss.
B · Check the feet every day and never walk barefoot
Peripheral neuropathy and poor circulation mean injuries can go unnoticed. Daily inspection, well-fitting footwear, checking water temperature and regular podiatry help prevent ulcers.
C · Report it; this is hand-foot syndrome and treatment may need changing
Palmar-plantar erythrodysaesthesia (hand-foot syndrome) is common with capecitabine and fluorouracil. It is graded and may need a treatment break or dose reduction; emollients and avoiding heat and friction help.
A · The infusion site, the rate and volume remaining, and the patient's symptoms
Nurses check that the infusion is running as planned, the site has no redness or swelling, and the symptoms are controlled. Problems are acted on quickly to avoid distress.
B · Give several deep-cough samples, often first thing in the morning
Usually a series of three sputum samples is collected, at least one in the early morning, when secretions have pooled overnight. The patient rinses with water (not mouthwash), takes deep breaths and coughs deeply to bring up sputum, not saliva. Airborne precautions are used. Sputum Culture · MedlinePlus (NIH) (2024); Acid-Fast Bacillus (AFB) Tests · MedlinePlus (NIH) (2025)
C · "I must not eat anything for 12 hours before the HbA1c test"
HbA1c reflects glucose attached to hemoglobin over about 2–3 months, so fasting is not needed. Lower HbA1c values are linked to fewer microvascular complications, and home glucose monitoring is still useful for day-to-day decisions. The A1C Test & Diabetes · NIDDK (NIH) (2018); Hemoglobin A1C (HbA1c) Test · MedlinePlus (NIH) (2025)
PaCO₂ = 26 mmHg
