LPN/LVN
C · Slow to 100–120/min and allow full chest recoil
High-quality adult CPR uses a rate of 100–120/min, a depth of 5–6 cm, full recoil after each compression and minimal interruptions. Leaning prevents the chest from re-expanding, so the heart fills less. Compressors should swap about every 2 minutes to avoid fatigue.
D · Cloudy fluid, many neutrophils, low glucose and high protein
Bacterial meningitis usually gives turbid CSF with raised neutrophils and protein and low glucose. Viral meningitis typically shows clear fluid with lymphocytes and normal glucose.
A · A patient on insulin for DKA whose potassium is now 2.9 mmol/L
Insulin drives potassium into cells, and a potassium of 2.9 mmol/L during DKA treatment can cause life-threatening arrhythmias. The insulin rate and potassium replacement need urgent review under the protocol. The other patients are stable.
A · Air can be heard even when the tube is in the lung
Auscultation of injected air cannot distinguish the stomach from the lungs or oesophagus, and misplaced tubes have caused deaths. pH testing of aspirate or X-ray is used instead.
D · Discuss reducing the fluid rate with the team
An output of about 1.7 mL/kg/h suggests over-resuscitation. Excess fluid ("fluid creep") worsens oedema and can cause pulmonary oedema and compartment syndromes, so the rate is reviewed and reduced under the burn protocol.
A · Magnesium
Hypomagnesaemia increases renal potassium loss and makes hypokalaemia hard to correct. Magnesium must be replaced as well.
D · Metastatic spinal cord compression
Back pain with neurological symptoms in a patient with cancer suggests metastatic spinal cord compression. It is an oncological emergency needing urgent MRI and treatment to preserve function.
B · A baseline HIV test
After an occupational exposure, the wound is washed and the incident is reported at once. Baseline tests for HIV, hepatitis B and hepatitis C are taken, and post-exposure prophylaxis is started as soon as possible if indicated, ideally within hours.
A · Pernicious anemia, treated with B12 injections
Pernicious anemia is an autoimmune condition in which antibodies block intrinsic factor or destroy the cells that make it, so B12 is not absorbed. Treatment is lifelong B12, usually by injection (very high oral doses are an alternative). Pernicious anemia · MedlinePlus (NIH) (2026)
Osm = 303 mOsm/kg
海面での肺胞気酸素分圧は PAO₂ = FiO₂·(760 − 47) − PaCO₂/0.8 mmHg で、A–a 勾配は PAO₂ − PaO₂ です。
