Using the Emergency Severity Index, which level fits a 70-year-old with chest pressure that suggests a heart attack, but normal vital signs?
D · A group A streptococcal throat infection
Rheumatic fever is an autoimmune reaction weeks after group A streptococcal pharyngitis. It can permanently damage heart valves, most often the mitral valve. Prompt antibiotic treatment of strep throat prevents it.
B · Intravenous calcium gluconate or chloride
Severe hyperkalaemia with ECG changes is an emergency. IV calcium stabilises the myocardium within minutes. Insulin with glucose and nebulised salbutamol shift potassium into cells, and the cause (here spironolactone and the ACE inhibitor) is stopped. Cardiac monitoring is essential.
C · Skin cancer
Transplant recipients have a much higher risk of skin cancers, especially squamous cell carcinoma, because of immunosuppression. Sun protection and regular skin checks are important.
B · Treat it as very high, check ketones and escalate
"HI" means the glucose is above the meter's upper range. In a drowsy patient this may be DKA or HHS. A laboratory glucose, ketones, observations and urgent medical review are needed.
D · It forms a protective coating over the ulcer base
Sucralfate binds to the ulcer base and forms a barrier against acid and pepsin. It works best on an empty stomach and can reduce the absorption of other drugs, so doses are separated.
C · Necrotising fasciitis
Necrotising soft tissue infection spreads rapidly along fascial planes and can cause septic shock. It is a surgical emergency needing urgent debridement, IV antibiotics and sepsis care.
B · Constipation
Opioids slow gut movement, and constipation persists with long-term use. A regular laxative is usually prescribed together with the opioid.
C · To shift potassium into cells; blood glucose must be monitored for hypoglycaemia
Insulin drives potassium into cells, temporarily lowering serum levels. Glucose is given to prevent hypoglycaemia, but glucose monitoring is still needed for several hours.
B · From the right arm
Venipuncture, blood pressure readings and IV lines are avoided in the arm with an AV fistula or graft, to prevent damage, clotting and infection of the access. Only the dialysis team uses the access, for dialysis.
B · Disseminated intravascular coagulation (DIC)
In DIC, widespread clotting consumes platelets and clotting factors, so PT and aPTT are prolonged and fibrinogen falls, while clot breakdown raises the D-dimer. ITP lowers platelets only, and hemophilia A prolongs only the aPTT.
PaCO₂ = 17 mmHg
AGc = 12.25 mmol/L
B · Level 2
Possible acute coronary syndrome is a high-risk situation, so the patient is ESI level 2 and should be seen very soon. Level 1 is kept for patients who need an immediate life-saving intervention.
