A patient coughs hard after abdominal surgery, and loops of bowel appear through the opened wound. What does the nurse do first?
During JumpSTART triage, a child is not breathing after the airway is opened but has a palpable pulse. What is the next step?
D · It can cause bronchospasm by blocking beta-2 receptors
Beta-2 receptors in the airways mediate bronchodilation. Blocking them can trigger severe bronchospasm in asthma and also blunts the effect of salbutamol. Cardioselective beta-1 blockers are safer but still used with caution.
C · A low-dose opioid with a hand-held fan and calm positioning
Low-dose oral opioids can ease refractory breathlessness in advanced disease. A fan blowing air across the face, upright positioning, breathing techniques and anxiety management also help. Oxygen is used only if it relieves symptoms or corrects hypoxaemia.
D · A beta-blocker alone can cause a dangerous rise in BP
Blocking beta receptors alone leaves catecholamines acting on alpha receptors, causing severe vasoconstriction and a hypertensive crisis. Alpha blockade, such as with phenoxybenzamine, comes first, and a beta-blocker may be added later for tachycardia.
A · "As long as I take my tablets, I can swim alone."
People with epilepsy should never swim alone, even if their seizures are well controlled. Showers are safer than baths. Telling friends about first aid and keeping a seizure diary are good practice.
D · To exclude significant arterial disease
ABPI compares ankle and arm systolic pressures. A low ABPI suggests arterial disease, where compression could cause ischaemia, so assessment is needed before compression therapy.
A · To rule out bleeding in the brain
A CT scan distinguishes haemorrhagic from ischaemic stroke. Clot-busting drugs are only given when bleeding has been excluded and other criteria are met.
A · Cardiac tamponade
Hypotension, raised JVP and muffled heart sounds (Beck's triad) suggest cardiac tamponade. Blood in the pericardial space compresses the heart and needs urgent drainage.
A · Glucagon by injection, or IV dextrose as prescribed
A patient who cannot swallow safely must not receive anything by mouth because of the risk of aspiration. Glucagon (injected or intranasal) or IV dextrose raises the glucose; once awake and able to swallow, the patient is given oral carbohydrate and a snack. Hypoglycemia · MedlinePlus (NIH); Low blood sugar · MedlinePlus (NIH) (2026)
Alkalosis · StatPearls (NCBI Bookshelf) (2024)
HCO₃⁻ = 29.6 mmol/L
D · Cover the wound with sterile dressings moistened with saline
Evisceration is a surgical emergency. The nurse covers the organs with sterile saline-moistened dressings, stays with the patient, positions them to reduce strain and calls the surgeon at once.
A · Give five rescue breaths; if breathing starts, tag the child red
In JumpSTART, an apneic child with a pulse gets five rescue breaths, because the cause is often respiratory. If breathing starts the child is immediate (red); if not, the child is expectant.
