A · Raised above the level of the heart when resting
Elevating the legs above heart level reduces venous pressure and oedema. Compression therapy, walking and avoiding long periods of standing also help. By contrast, patients with arterial disease often find that lowering the legs eases pain.
B · Stay for observation, then go home with an adrenaline auto-injector and teaching
Biphasic reactions can occur hours after the first reaction resolves, so patients are observed for a period determined by severity and local guidance. Before discharge they receive adrenaline auto-injectors with training, a written plan and referral to an allergy service.
D · Vitamin B12 level
Long-term metformin can lower vitamin B12 absorption, causing anaemia and neuropathy that may be mistaken for diabetic neuropathy. B12 levels are checked, especially when symptoms appear.
A · That the catheter is not blocked or kinked
A sudden drop in output after transplant may be caused by a blocked catheter, often from clots. The nurse checks the tubing and may flush the catheter per protocol, and then escalates urgently, because vascular thrombosis or rejection are also possible.
B · Escalate urgently, as immune-related colitis is likely
Severe diarrhoea on checkpoint inhibitors suggests immune-mediated colitis, which can perforate. It needs urgent assessment, infection testing and often high-dose steroids, not only antidiarrhoeals.
A · Their sense of thirst is often reduced
With age, thirst is blunted, kidney concentrating ability falls and total body water decreases. Cognitive or physical problems can also make it harder to drink.
D · Liver damage develops later, and the antidote works best when given early
Paracetamol overdose may cause few early symptoms, but liver failure can develop over days. Acetylcysteine is most effective when started early, guided by timed paracetamol levels.
B · Kidney function (creatinine clearance)
Dabigatran is excreted mainly by the kidneys, so poor kidney function raises the drug level and bleeding risk. Kidney function is checked before starting and regularly afterwards. Routine INR monitoring is not used for direct oral anticoagulants.
B · Insulin delivery failure; inject a correction dose by pen and change the set
A high glucose that does not respond to a pump bolus, with rising ketones, suggests a kinked, blocked or dislodged cannula. Because pumps use only rapid-acting insulin, DKA can develop within hours. The usual advice is to give a correction dose by pen or syringe, change the infusion set, check glucose and ketones often, and seek help if ketones keep rising or vomiting starts.
PaCO₂ = 26 mmHg
D · Relieve pressure on the area by changing position regularly
Non-blanching redness is a stage 1 pressure injury. Relieving pressure through repositioning and support surfaces is the priority; massage can damage tissue further.
